Tanta University Faculty of Nursing Medical surgical Nursing Department Neurology Area Case Study about: "Stroke " Prepared by: Sabreen Mohamed Group: C6 Section: Neurology Under Supervision: Prof.Dr: Seham Ahmed Dr: Enas Ahmed Mr Magdy Ibrahim is Male patient ,has 65 years,he is Married, he is smoker,he was admitted to tanta hospital in Neurology Department, is referred from outpatient clinic of tanta hospital At 12/11/2024 Suffering from Suddenly stopped in the right side of body,spasm,cramps.He diagnosis with stroke. the patient suffered from chronic disease: (diabetes, Hypertension). The patient is conscious, Right limb sensation and strength is abnormal, also the movement is abnormal, in addition to he is lying in bed and cannot perform basic activities such as walking, drinking, eating and wearing clothes. The patient diet is abnormal ,He has nasogastric tube. Patient Current investigation that include: CBC,CRB,INR,hb. vital signs:Temperature.37^c,pulse89b/m Respiration 20c/m ,blood pressure : 130/70mm.hg. Patient Current medication that calxan 40U Q12H,aspiren 100m.g,rengar 500 Q24H,Cefotax 1g Q12 H, Inhalation sessions (pulmacort,Atrovent), protofex 1ampual Q12H,insulin according prescribed ,Danset ampual Q12H ,cal.preg 2tables Q8H. Assessment of level Glasgow coma scale: Eye opening 4 spontaneous , Verbal response 2 incomprehensible sound , motor Response 2 assumes extensor posturing. He Suffering from problems in movement related to weakness in the right side, Vomiting, dyspnea Difficulty in breathing, Difficultly in swallowing,self care deficit (bathing, Grooming, dressing, feeding)related to strok.
