Cancer Plus Plan Overview Non-medical plan Issue age is 18 to 64 years Expiry date is the policy anniversary nearest the life insured’s 75th birthday or on the 20th anniversary of the issue date, if later. The 20 years if later is only for ages 56 – 64 since the policy would be in force for 20 years at age 75. Plan Overview Maximum benefit payable: on the diagnosis of most forms of cancer. (living benefit) ◦Ages 18 – 50 $10 million ◦Ages 51 – 60 $9 million ◦Ages 61 – 64 $5 million Plan Overview Provides a living benefit in lump sum payout in the event the life insured is diagnosed with cancer (based on definition) If no claim has been made before the Expiry Date and all Basic Premiums due have been paid, the benefit payable on the Expiry Date is the total of all basic Benefit Proceeds Benefits are payable in the following circumstances: The sum insured becomes payable on the occurrence of the first diagnosis of the Life Insured with Cancer (see definition) The benefit payable on death of the Life Insured occurring on or before the Expiry Date is the return of all Basic Premiums paid less any indebtedness Benefits  The sum insured less any indebtedness is paid on the first diagnosis of cancer provided that: The life insured was not previously diagnosed with cancer Where the diagnosis occurs during he 1st 90 days of issue date, the benefit is the return of all premiums Benefits Free Cancer Screening The life insured has the facility to access free cancer screening at the Jamaica Cancer Society after the 2 nd policy year and every two years thereafter. No Claim Bonus A full refund of all premiums will be paid upon the death of the life insured, provided the policy is in force and all premiums have been paid. Benefits Encashment\Surrender Value This policy does not accumulate cash values or surrender value Termination The policy will cease to be in force if any premium is outstanding for longer than 30 days or upon the payment of any benefit. Suicide If the Life Insured, whether sane or insane, commits suicide before the date which is two years after this policy’s issue date, or before the date which is two years after the date of any reinstatement, the company’s liability under the policy will be limited to the amount paid to the company in the form of basic premiums, the amounts being those paid under the policy since the issue date. Definition of Cancer Cancer, means the uncontrolled growth and spread of malignant cells and invasion of tissue as evidence by definite histology. It includes leukemia, myloma, lymphoma, Hodgkin’s Disease, malignant melanomas and squamous cell carcinoma but excludes non-invasive cancers in situ, kaposi’s sarcoma.
