This is an online document which can be accessed by four people (chapter worker, RD, FD, and ND). BEGINNING STATUS Describe briefly the status of the chapter as the academic year begins. To the best of your knowledge, how long has this group been pioneering/ developing/ established? Where are most of the students in terms of our IVCF discipleship process? Enumerate your resources. In the monitoring table input your baseline numbers. Start writing here. 2. YEAR-END GOALS & WORKER’S ACTION PLAN Fill in the monitoring table in Part 3 below with your year-end targets for each indicator. Then enumerate steps you will take each quarter in order to meet those targets. For example: in the 1st Quarter you have mostly believers, so in order to help them move on to become disciplers, your 1st Qtr action plan is: Implement Come & Grow. Every reporting time you may edit this portion. But do not delete any text. Midyear Narrative: During my second week in Iba, we held a Campus Evangelism event at PRMSU with Kuya Charis, using the "Kabutihan Test." This was followed by another session the following week, where I joined the students. It was an exciting experience as it challenged us to approach individuals and groups in a thoughtful and engaging way. After sharing the gospel, we invited them to our semester-end event, "Unshakeable," and collected their names and contact information for follow-up. Midyear Narrative: The first step I took was getting to know the student leader, both in person and through Messenger chats. I also visited their local church to engage with them more deeply and to understand how they carry out their ministry in their local church. Midyear Narrative: We conducted a workshop for student leaders on how to evangelize, with the guidance of our adviser, Kuya Sam. A classroom was allocated for the workshop. During the session, we also practiced how to approach different scenarios and handle various student attitudes. Additionally, we reminded the student leaders to consistently follow up with their small groups. How did you steward your resources, care for your workers/volunteers/partners, and keep the vision alive? 4. INDIVIDUAL TRANSFORMATION STORIES Compile your transformation stories here. Indicate if it is a conversion story (a seeker becoming a believer) or a transformation story (a believer maturing through the discipleship process). Indicate also the name and date of stories. COMMUNITY ENGAGEMENT STORIES Compile your community engagement stories here. Share about how your chapter - not individual members but the chapter - is engaging its campus, communities, or the nation. Examples: your chapter initiated a friendly dialogue with the local atheist club, your chapter noticed the perennial problem of littering and had a Clean-Up Day, your chapter issued a public statement regarding a new Senate bill, or your campus ran an anti-bullying campaign. ANALYSIS Provide thorough analysis for the quantitative and qualitative developments described in PARTS 3 - 5. Explain the reasons why your chapter is on track or NOT on track towards achieving the targets you set for the year. Then go back to Part 2 and rework your action plans as necessary. Midyear Analysis: [box extends to accommodate longer text] Year-End Analysis: As I started my assignment here in Iba, I noticed that the students are putting in their best effort to implement the plans they made the previous month. However, conflicts arose during the processing of their papers for the semester-end event, which led them to resort to Plan B. Fortunately, RCVF has a partner church that can collaborate with them for the event. The students really need someone to check on them and ask how they are doing. From the previous mistakes, we learned that important tasks, like processing documents, should receive proper attention. As a result, early preparation is now included in the plan to avoid similar issues in the future. To be honest, I’m not entirely sure if they are on track to achieving their goals. When I speak with the leaders, their usual response is that they are “extremely busy with academic reports, theses, projects, other organizations, and more.” 7. SUPERVISOR’S FEEDBACK Your supervisor will initially write down her feedback to your report. S/he may revise her written feedback here after she has talked with you about her initial feedback. Midyear Feedback: You have a good number of seekers at the start of the year. Talk with the leaders and have these seekers engage with the Gospel (same- gender) if possible with you so you can mentor a student leader how to engage their fellow students to be believer of Jesus.
