NEWGIZA UNIVERSITY BDS8235 Clinical partial prosthodontics: Non-metallic partial dentures Aim and Objectives NEWGIZA UNIVERSITY Objectives: • On completion of this lecture, the student should have: • An understanding of the range of different designs and materials used in partial dentures • An understanding of how and when to used non-metal partial dentures Topics to be covered: • Indications of non-metal partial dentures • Materials used (flexible or normal acrylic resin) • Special considerations for design of denture • Denture components and how different they are from a framework denture • Advantages and disadvantages of such dentures (non-metal base or totally non-metal denture) Definition • Removable partial dentures that are designed with out a metal framework to be used for short intervals and considered as a part of a total prosthodontic treatment are termed temporary. Indications • To restore Appearance • Space maintainer • Reestablishment of occlusal relations • Conditioning prosthesis • Interim restoration during treatment • Patient Training • When cost is a prime requisite Forms & Types All acrylic PD Spoon Denture immediate Denture Non-metallic Partial Denture Treatment PD Interim PD Transitional Denture All Acrylic Partial Denture All Acrylic Partial Denture • They consist of an acrylic denture base, artificial teeth and wrought wire clasps or cast clasps. All Acrylic Partial Denture ADVANTGES  Less expensive DISADVANTAGES • Lower strength  Light in weight.  Simpler in design  easily constructed. • Less patient tolerance. • Poor thermal conductivity.  A lost natural tooth can be easily added.  Easily repaired and relined.  Provides good esthetics • Does not provide efficient tooth support or indirect retention when needed. Design Considerations Retentive elements Major connector Bracing Support elements Retentive Elements • Retention is mainly achieved by wrought wire clasps which may vary in shape and diameter • Also retention my be achieved by acrylic extension undercuts in the inter proximal Wrought wire clasps embedded in the acrylic is used for retention of acrylic partial dentures Support elements Wrought wire extensions could be used as support elements for the acrylic dentures Support elements Wrought wire could be used for retention and support for the acrylic dentures Major connector  Full palatal coverage Or Lingual plate Bracing & Reciprocation Elements • major connector provides bracing through the part contacting the lingual height of contour Clinical Considerations Insertion& Occlusal Adjustment • During insertion special care should be considered for the interproximal areas which might interfere with proper seating of the denture. • Over reduction of interference should be avoided to prevent food entrapment Flexible partial dentures • These are dentures constructed from flexible thermoplastic resin materials. • Flexible partial dentures have the following benefits: They have no unsightly metal parts, as they don't require metal clasps. They're more comfortable to wear and for the wearer to chew and eat due to the thin and flexible thermoplastic resin material. They're less likely to cause irritation Spoon Denture The best treatment is bridge or implant Indications Of Spoon Denture Incompletely calcified roots (young age) As a space maintainer Allowing alveolar resorption to take place after extraction Spoon Denture Transitional Partial Denture Transitional Partial Denture This type of denture is used to accommodate multiple addition of natural teeth planned to be extracted Impression and mounting of the existing denture Clasp is removed Teeth to extracted is removed from the cast Wax up is completed and artificial teeth is added Wax is processed and denture is finished and polished ,ready for insertion after teeth extraction Treatment Partial Denture Treatment Denture Indications: Assessment for changing the Vertical Dimension Alteration of vertical dimension / occlusion Conditioning Of tissues before definitive prosthesis Treatment Partial Denture Immediate Partial Denture Advantages  The remaining natural teeth act as an excellent preextraction record  Preserve patient appearance.  The patients can better tolerate the transition from dentulous to edentulous state, promotes better ridge form  The functions of speech and mastication are sustained. Advantages • Decrease the physiological trauma to patient. • Provides a guide for the V.D.O. • Less interruption of business and social life • Provides a dressing to protect the wound, blood clot from food and tongue action → promotes healing.
