Acute Abdominal Emergencie s Its more than just gas Abdominal Contents Solid organs bleed Hollow organs dump their contents causing infection Causes of Abdominal Pain The “-itis” Causes Peritonitis-(Inflammation of the abdominal wall) Appendicitis-(Inflammation of the Appendix) Starts around the bellybutton and radiates down the right side of abdomen. Rebound tenderness Cholecystitis-(Inflammation of the Gallbladder) Referred pain to the shoulder The “-itis” Causes Pancreatitis-(Inflammation of the Pancreas) Gastroenteritis-(Inflammation of the GI Tract) Hepatitis-(Inflammation of the Liver) Cholecystitis The 7 F’s Flatulant Fat Female 40 Fertile Fair Skinned Freckled GI Bleeding Bleeding somewhere in the GI tract Can be quite stinky Note condition of blood, bright, dark or coffee grounds and where its coming from (Mouth or butt) Esophageal Varices Erosion of esophageal tract Generally due to alcoholism Bleeding can be severe May run into Airway and Breathing issues Abdominal Aortic Aneurysm Just like thoracic aneurysm, but in the abdomen Same signs and symptoms, just in the abdomen May be able to feel pulsating mass in skinny folks Primary Survey General Impression Note patient positioning and level of pain Level of consciousness Chief Complaint Patients generally will tell you exactly where it hurts prior to OPQRST Primary Survey Airway If patient talking, airway is patent B Patient may be breathing shallow due to pain C Note if radial pulse is present, skin signs SAMPLE Hx. Do a good OPQRST May lead to narrowing down a diagnosis Vital Signs and Focused Secondary Note that pain stimulates sympathetic nervous system May have fast breathing and pulse, poor skins Differential diagnosis is important!
