Without precisely following the correct procedures in sample reception, the quality of results is not assured,. Research statistics has shown that most laboratory errors take place in the pre- analytical stage, of which sample reception is a central part. It is therefore crucial that all the procedures are followed meticulously. Departments of A Medical Laboratory 4 Haematology Biochemistry Immunology & serology Microbiology Urine, Stools, Blood, Body fluids Pathology Genetics Lab Request Form 5 Information should include: Patient name and ID (Double Identification) Age Gender Hospital Room & Department Date and time of collection Sample type and site Person who collected the sample A list of the tests that need to be performed Provisional diagnosis Referred Physician Samples that arrive at a laboratory without clear labelling are generally rejected. Sample Rejection 6 Incomplete request form Ambiguous Identification Unlabeled, Wrongly labeled samples Mis-matched samples Improper container Improper temperature Leaking container Insufficient quantity Dried sample Prolonged transport time Registry & Labeling 7 These follow receiving and accepting the sample. A patient register can be maintained either on paper or electronically. There are many Laboratory Information System (LIS) software packages. Sample labeling: when the sample is introduced in the register the sample is labeled. This creates the traceability: sample portions are made traceable to the primary sample by using the same label on all the portions. This in turn ensures that all the sample information is properly recorded. 8 Samples are then introduced to the specialty area or bench according to the type of sample and required tests for further testing. Sample reception is a central part in the pre-analytical stage. It is therefore crucial that all the procedures are followed meticulously, and that all the controls are performed thoroughly and at all times. 10 Sample processing: Review of requests by authorized personnel. Procedure for processing urgent samples. Procedure for dealing with verbal requests (ensuring that all the relevant information is collected by filling-out a Request Form by the laboratory based on the verbal request).
