## Surgery ### Emergencies in Hiatal Hernias * Bleeding * Infarction * Perforation ### Emergency: Higher mortality upon the diagnosis of Hiatal Hernia, we repair it surgically on an elective basis, because: * 1% mortality rate on elective * 6 out of 21 patients in one study (30% very high mortality rate) ### Transthoracic vs Trans-abdominal (both may be Laparoscopic or Open) It is more advised to perform a laparoscopic procedure instead of open except, if and when there is competence (surgeon), operating tools, which can penetrate general anesthesia ### Limitation of Transthoracic: Diaphragmatic repair may be performed but not fundoplication
