Study Notes
Canine Hypothyroidism and Corneal Lipidosis
- Hypothyroidism is the most common canine endocrinopathy, estimated at 0.2%-0.8% prevalence.
- It often results from primary acquired thyroiditis, but other types exist.
- Clinical signs include systemic and ocular manifestations, including keratoconjunctivitis sicca, arcus lipoides corneae, lipemic aqueous, ocular hemorrhage, lipemia retinalis, and bullous retinal detachment.
- Corneal lipidosis, specifically arcus lipoides corneae, involves lipid deposition in the cornea.
- Arcus lipoides corneae often manifests bilaterally and is associated with hypothyroidism in dogs.
Case Summary
- A 4-year-old spayed female Sheepdog-Poodle presented with visual impairment due to corneal opacification.
- The opacification and impairment had progressed over 2 years.
- Initial presentation showed bilateral, severe arcus lipoides corneae.
- Bilateral, symmetric, and multifocal bullous retinal detachments were noted.
- Hyperlipidemia (likely related to primary acquired thyroiditis) was detected.
- Unilateral lamellar keratectomy improved corneal clarity and visual function.
- Bilateral retinal detachment and hyperlipidemia resolved following thyroxine supplementation.
- Ocular symptoms improved, with the treated eye showing better clarity and the untreated eye remaining static.
Surgical Procedure
- Lamellar keratectomy was performed on the affected eye (OD).
- A 6400 Beaver blade was used to perform the circumferential lamellar keratectomy, avoiding ~1 mm from the limbus.
- The depth was less than 50% of total corneal thickness.
- A Martinez corneal dissector was used to continue the dissection.
- Some clear cornea was preserved centrally.
- Amnion membrane was applied to the cornea, secured with sutures, and covered with a bandage contact lens.
- Partial temporary tarsorrhaphy was done.
Post-Operative Care
- Topical and systemic medications were prescribed post-operatively, and systemic medication was continued with monitoring.
- Ofloxacin, tobramycin, and topical lubrication were administered.
- Carprofen and gabapentin were also given.
Histopathology
- Histopathological analysis of the excised cornea exhibited severe chronic granulomatous keratitis, stromal fibrosis, and cholesterol clefts.
