Urinary & Bowel Medications

Colinergic Agonist

  • Drug: Bethanechol
  • Indications: Urinary Retention, muscle tone in GI, heartburn
  • Contraindications: Hyperthyroidism, asthma, CAD, epilepsy, Parkinson's, PUDI, bowel obstruction
  • Adverse effects: Decreased BP, syncope, asthma attack, bronchoconstriction, overdose, increased heart rate and BP, respiratory depression, bronchospasm, headache, dizziness
  • Note: Antidote - Atropine

Anti-colinergic

  • Drugs: Darifenacin, oxybutynin, tolterodine
  • Indications: Urge to void, urinary incontinence, urgency, neurogenic bladder
  • Contraindications: Glaucoma, urinary retention, BPH
  • Adverse effects: Blurred vision, urinary retention, constipation, dry mouth
  • Note: Pilocarpine can treat glaucoma, perform oral care twice a day and take fluids, use sugar free hard candy or gum

Urinary Analgesic

  • Drugs: Phenazopyridine
  • Indications: UTI, pain, frequency, urgency
  • Adverse effects: Bright orange urine, don't break, chew, or crush

Anti-Diarrheals

  • Drugs: Diphenoxylate w/ atropine and loperamide, also bismuth subsalicylate
  • Indications: Diarrhea
  • Contraindications: C. Diff, infectious diarrhea
  • Adverse effects: Bismuth subsalicylate can cause reye's syndrome, dark tongue, and bleeding, contact provider for prolonged or worse diarrhea, fever, bloody stools, and monitor for dehydration

Laxatives

  • Types: Bulk forming (psyllium methyl-cellulose), emollient/stool softener (docusate, mineral oil), hyperosmotic (lactulose, polyethylene glycol, sorbitol, glycerin), saline (magnesium salts, fleet enema), stimulant (bisacodyl, senna), opioid antagonist (methylnaltrexone, alvimopan)
  • Indications: Constipation
  • Contraindications: Fecal impaction, intestinal obstruction, undiagnosed abdominal pain, saline use in renal disease
  • Adverse effects: Impaction, obstruction, abdominal discomfort and cramping, magnesium salts cause electrolyte imbalance
  • Note: Interferes with the absorption of other medications, increase fluids and mobility, take with 8oz of water for bulk forming, safe for long term use as long as taken with fiber, electrolyte supplements for hyperosmotic risk, dependence likely for stimulants
