Accident/Incident Resulting in a Critical Injury or Death
- Supervisor determines if injury is critical using Standard Operating Guideline S-CRIT – Critical Injuries
- In case of critical injury or death, Supervisor must:
- Preserve evidence and maintain scene integrity
- Record details of original scene if it's necessary to disturb it
- Secure the scene to prevent secondary accidents
- Immediately notify on-duty Division Chief/Platoon Chief and on-duty Designated Officer
First Aid Incident Only
- Supervisor's actions include:
- Providing first aid if necessary
- Providing WSIB Form 6 to employee for potential future medical treatment
- Conducting a detailed investigation with Health & Safety Consultant
- Completing a Supervisor's Report of Injury/Incident and forwarding a copy to District Chief
- Having witnesses complete an Incident Observation Form
Supervisor Responsibilities for a Medical Aid Injury/Illness
- Supervisor ensures employee receives immediate first aid treatment as necessary
