Officer of Note
- Typically the Company Officer aboard the first arriving apparatus at an incident, unless otherwise determined by the Incident Commander.
- Responsible for completing the main report for an incident, called the Incident Report.
- Incident Report is required for every emergency dispatch, even if all apparatus are cancelled while en-route.
Incident Report
- Completed by the Officer of Note, unless the District Chief is the Officer of Note for larger incidents (Second Alarm or greater).
- District Chief's Incident Report must be reviewed by the Platoon Chief.
- Platoon Chief lists the name of the Officer of Note in their Narrative for larger incidents.
Unit Report
- Completed by all Officers (except the Officer of Note) to indicate the actions of themselves and their crew for all incidents.
- Does not include false alarm incidents, unless the Incident Commander requests a report.
- Officer's Narrative in the Unit Report is required for incidents where they assume Command.
Exposure Reports
- Completed by the Officer of Note, who may assign the task to any Officer involved with the exposure.
- Incident times are copied over to the new report, but Officer must complete the report in detail, including exposure address and apparatus information.
Community Risk Reduction (CRR) Referrals
- Company Officers observing Fire Code violations report to the CRR Division using the Fire Prevention Notes box and Narrative fields of the Incident or Unit Report.
- Separate Narrative is required for CRR referrals, including apparatus number and platoon.
- Serious Fire Code violations require notification of a District Chief and appropriate action.
The Arson Prevention Program for Children (TAPP-C) Referrals
- Operations personnel observing conditions requiring TAPP-C intervention refer incidents using the Fire RMS Incident or Unit Report.
- Company Officers observing cases requiring TAPP-C intervention report to TFS Public Education using the Fire Prevention Notes box and Narrative fields.
- Separate Narrative is required, including apparatus number and platoon, but excluding specific details of the incident, particularly names of youth individuals.
