    Study Notes

    Transition in Nursing

    • Transition is a movement or development from one stage or style to another, specifically from student to registered nurse (RN) and from the school/university setting to the workplace.
    • It is a complicated process during which many changes may happen at once.

    Benner Stages of Learning

    • Stage 1: Novice nurse with few experiences and skills learned by rote, usually occurring during nursing education.
    • Stage 2: Advanced beginner who performs adequately and makes some judgment calls based on experience, most novice nurses enter the workforce during this stage.
    • Stage 3: Competent nurse who foresees long-range goals and masters skills.
    • Stage 4: Proficient nurse who views the whole situation rather than parts and develops solutions.
    • Stage 5: Expert nurse with instantaneous intuition and decision-making.

    Reality Shock

    • Reality shock occurs in novice nurses when they become aware of the inconsistency between the actual world of nursing and that of nursing school.
    • There are four phases of reality shock:
      • Honeymoon Phase: Everything is as imagined during orientation with former school friends or other new graduates.
      • Shock (Rejection) Phase: Daily assignments begin, and the nurse experiences conflicting viewpoints, discomfort, and rejection of the new environment.
      • Recovery Phase: The nurse understands the new culture, and tension and anxiety decrease.
      • Resolution Phase: The nurse positively adjusts to the new environment, grows as a person and professional, and develops self-confidence.

    Causes of Reality Shock

    • Fear, lack of confidence, total responsibility
    • Unrealistic expectations of self and coworkers
    • Fatigue, role conflict, and undefined expectations
    • Lack of resources and support
    • Dealing with physicians and other healthcare workers
    • Lack of organizational skills/new situation
    • Heavy workload, frequent interruptions

    Strategies to Ease Transition

    • Interpersonal skills: Develop relationships with healthcare team members, listen, ask questions, and avoid distractions.
    • Clinical skills: Possess basic knowledge of nursing skills, develop skills through experience and practice.
    • Role preparedness: Plan, prepare, perform, and prioritize tasks effectively.
    • Organizational skills: Develop time management skills, use report sheets, and contact former nursing instructors for guidance.
    • Delegation skills: Use body language effectively, delegate tasks, and provide support to coworkers.
    • Self-mentoring: Learn from experiences, develop problem-solving skills, and ask questions.
    • Self-confidence and self-esteem: Believe in oneself, take sensible risks, and take care of oneself.

    Additional Strategies

    • Biculturalism: Integrate school values and work environment values to improve self-image.
    • Role models and mentors: Learn from experienced nurses, set realistic goals, and receive guidance.
    • Preceptorships: Participate in preceptor programs to socialize into the profession and ease transition.
    • Clinical skills development: Ask to observe or assist experienced nurses with procedures, prioritize skill development.
    • Interpersonal skills development: Attend unit meetings, volunteer for committees, and take part in professional organizations.
