The Seven Rights of Medication Administration
- The seven rights of medication administration are: Patient, Medication (Drug), Amount (Dose), Route, Time, Allergies, and Documentation.
- Patient identification is crucial, and can be done through scanning wrist IDs and asking the patient to self-identify.
- Medications must be ordered correctly and undergo three label checks. Blister packs should have the name, name of the medication, and expiration date.
- Medications should be dispensed or poured, and the packet should be double-checked to ensure it is correct.
- Look-alike or sound-alike drugs should be avoided, and brand names should be written in capital letters while generic names start with lowercase letters.
- Proper labeling is important, and medications should not be returned to the drug bottle or left at the bedside without a doctor's order.
- The route of administration must be specified in the doctor's order, and the most common route is oral. Other routes include Gtube, transdermal, inhalation, nasal, eye, ear, rectal.
- Dosage calculations are vital, and measuring devices such as cups, droppers, oral syringes, and dosing spoons should be used.
- Medications should be administered at the specified time/frequency as ordered by the doctor, and abbreviations and standard times should be understood.
- Documentation is important and should include patient information, drug details, route, dose, date and time, site of injection, and patient's response.
- Medication errors are reportable and can result in termination. They are monitored by organizations such as USP, FDA, and ISMP and can lead to malpractice.
- Different routes of administration include gastrostomy tube, nasogastric tube, oral, ear, eye, nose, vaginal, rectal, inhalation, sublingual, parenteral, and enteral.
- There are different types of drug dispensing systems, such as computerized systems, unit dose, blister pack, floor stock, and individual prescription. Drug names can be chemical, trade, brand, proprietary, generic, USP, or NF.
