Assessment

- Data is collected from various sources to identify potential health problems.
Categories Of Health Problems
- Health problems can be categorized into three main types: Health Deficit, Health Threat, and Foreseeable Crisis.
Health Deficit
- Represents a gap between the individual's current health status and their achievable health potential.
- Examples include diagnosed or undiagnosed illnesses, failure to thrive, disabilities, and permanent impairments.
- These deficits can encompass both transient (temporary) and permanent conditions.
Health Threat
- Refers to conditions that increase susceptibility to disease, accidents, or hinder personal development.
- Examples include family history of hereditary diseases, potential for cross-infection, accidents, unhealthy lifestyle choices, and poor sanitation.
Foreseeable Crisis
- Anticipated periods of significant stress or strain on individuals and families, requiring adjustments and potentially exceeding available resources.
- Examples include marriage, pregnancy, parenthood, divorce, job loss, menopause, and death.
Planning
- This step involves developing a care plan based on identified needs and capabilities.
- It includes prioritizing needs, setting goals, constructing a plan of action, devising evaluation parameters, and revising the plan as necessary.
Implementation/Intervention
- This step translates the care plan into action.
- It involves coordinating care services, utilizing resources, delegating tasks, monitoring health services, providing education, and documenting responses to interventions.
Evaluation
- This step involves assessing the effectiveness of the care plan and identifying areas for improvement.
- It includes evaluating care outcomes, conducting performance appraisals, estimating cost-benefit ratios, assessing for further problems, suggesting needed alterations, and revisiting plans for adjustments based on the evaluation.
