Secondary Hemostasis and Fibrinolysis
- Hemostasis consists of two stages: primary and secondary.
- Primary hemostasis involves the formation of an unstable platelet plug.
- Secondary hemostasis reinforces the unstable platelet plug by transforming soluble fibrinogen into insoluble fibrin, which seals the injured vessel and initiates fibrinolysis.
- Coagulation mechanism involves intrinsic pathway, extrinsic pathway, and common pathway.
- Coagulation factors assigned Roman numerals I to XIII and divided into three groups: Prothrombin, Fibrinogen, and Contact groups.
- The Prothrombin group (II, VII, IX, X) is vitamin K dependent and produced in the liver.
- The Fibrinogen group (I, V, VIII, XIII) is not found in serum because they are consumed during coagulation.
- The Contact group (XI, XII, PK, HK) is involved in the initial activation of intrinsic pathway and requires a negatively charged surface for activation.
- Most coagulation reactions occur on the surface membrane of activated platelets, which provide the critical phospholipid for coagulation.
