Surgical Wound Classifications
- Four main classifications: Class I, Class II, Class III, Class IV.
- Class I: Clean wounds, no inflammation or infection, no entry into respiratory, alimentary, or genitourinary tracts.
- Class II: Clean/Contaminated wounds, entry into tracts under controlled conditions, no signs of infection.
- Class III: Contaminated wounds, involve open traumatic injuries, major sterile technique breaks, or GI spillage.
- Class IV: Dirty/Infected wounds, existing infection, retained devitalized tissue, or perforated viscera.
Class I Wounds
- Clean operative wounds common in elective surgeries.
- Examples: Total Knee Arthroplasty, Elective Eye Surgery, Hernia Repair, Breast Surgery, Non-Traumatic Neurosurgery.
- Typically characterized by primary closure and possibly closed drainage systems.
Class II Wounds
- Clean/Contaminated wounds, where controlled entry into tracts occurs.
- Examples include Total Hysterectomy, Dilation and Curettage (D&C), Total Abdominal Hysterectomy, Gastrectomy, and Elective Appendectomy.
- Conditions in this class show no major aseptic technique breaches.
Class III Wounds
- Contaminated wounds presenting with fresh traumatic conditions.
- Include scenarios such as gunshot wounds, laparotomy with significant spillage, traumatic wounds without viscera perforation, and acute inflammation of organs without pus.
- Encounter major breaks in sterile technique or gastrointestinal spillage.
Class IV Wounds
- Dirty or infected wounds, including old traumatic injuries and current clinical infections.
- Characterized by retained devitalized tissue or delayed primary closures.
- Examples: Incision and drainage of an infected wound, total evisceration, and perforated viscera.
