Stages of Labor 1. Dilatation Stage (0-10 cm) - enlarging, opening, widening 1. True Labor - cannot be relieved by ambulation, pain at the back, difference: bloody SHOW 2. False Labor - intermittent, pain at abdomen, can be relieved by ambulation, Braxton Hicks, main difference: white SHOW Dapat ______ na patient; KVO (Keep vein open) Passenger - Placenta, Baby, Amniotic Fluid Cause of prolonged labor / painful labor: distended bladder and abdomen Transfer to DR when: transition phase (7-10) Transition to labor room;l: active phase (4-6) latent (0-3) 1. Delivery Stage - full dilatation to delivery of baby 1. bulging of perineum (area between vagina and anus) - for episiotomy (surgical incision of perineum) 1. perineorrhaphy (repair of perineum) 2. episiorrhaphy (repair of episiotomy) 2. Crowning - encirclement of presenting part 3. Rupture of BOW 1. normal (500) Amniotomy - shorten the second stage of labor Normal Amniotic Fluid - Normohydramniod Amniotic Fluid - straw or clear Amniosynth.

Amniotic Fluid

Normal Amniotic Fluid - Normohydramniod Amniotic Fluid - straw or clear Amniosynth.

shorten the second stage of labor

Delivery Stage - full dilatation to delivery of baby 1. bulging of perineum (area between vagina and anus) - for episiotomy (surgical incision of perineum) 1. perineorrhaphy (repair of perineum) 2. episiorrhaphy (repair of episiotomy) 2. Crowning - encirclement of presenting part 3. ______ of BOW 1. normal (500) Amniotomy

Rupture

Delivery Stage - full dilatation to delivery of baby 1. bulging of perineum (area between vagina and anus) - for episiotomy (surgical incision of perineum) 1. perineorrhaphy (repair of perineum) 2. episiorrhaphy (repair of episiotomy) 2. Crowning - encirclement of presenting part 3. Rupture of BOW 1. normal (500) ______ - shorten the second stage of labor Normal Amniotic Fluid - Normohydramniod Amniotic Fluid - straw or clear Amniosynth.

Amniotomy

Study Notes

Stages of Labor

  • Dilatation Stage (0-10 cm) involves enlarging, opening, and widening of the cervix.

True Labor vs. False Labor

  • True Labor:
    • Cannot be relieved by ambulation
    • Pain is located at the back
    • Bloody SHOW (a sign of labor)
  • False Labor:
    • Intermittent pain
    • Pain is located at the abdomen
    • Can be relieved by ambulation
    • Braxton Hicks contractions
    • White SHOW (a sign of false labor)

Preparation for Labor

  • Patient should be NPO (nothing by mouth) to prevent aspiration during delivery
  • KVO (Keep vein open) to maintain hydration

Key Concepts

  • Passenger refers to the placenta, baby, and amniotic fluid
  • Causes of prolonged or painful labor include distended bladder and abdomen
  • Transfer to delivery room (DR) when patient reaches the transition phase (7-10 cm)

Phases of Labor

  • Latent phase (0-3 cm): early labor
  • Active phase (4-6 cm): moderate labor
  • Transition phase (7-10 cm): advanced labor

Delivery Stage

  • Full dilatation to delivery of baby
  • Bulging of perineum indicates need for episiotomy (surgical incision of perineum)
  • Perineorrhaphy: repair of perineum
  • Episiorrhaphy: repair of episiotomy
  • Crowning: encirclement of presenting part
  • Rupture of BOW (bag of waters)

Amniotic Fluid

  • Normal amniotic fluid: normohydramnios, straw or clear color
  • Amniotomy: surgical rupture of membranes to shorten the second stage of labor
