Root Canal Anatomy

- Understand the two most commonly used classification systems (Vertucci and Weine) for root canal system and their limitations.
Root Canal Components and Morphology
- The root canal system can be divided into two parts: the pulp chamber and the root canal space.
- The pulp chamber is a cavity normally situated in the center of the crown and resembles the shape of the crown surface.
- The root canal space is found inside the radicular portion of the tooth.
Isthmus
- An isthmus is a narrow, ribbon-shaped communication between two root canals that may contain vital tissue, necrotic pulp, biofilms, or residual filling material.
- Isthmuses may present with different configurations and their prevalence is dependent on the type of teeth.
- Isthmuses can be commonly observed after root-end resection in cases referred for apicoectomy treatment.
Clinical Significance
- Unfilled isthmuses can be a limitation in nonsurgical treatment.
- Chemical agents that have the ability to dissolve organic tissue at fine levels can be used to surpass these limitations, often associated with ultrasonic activation.
