- It provides support, protection, movement, and blood cell production.
- It's categorized into axial (80 bones) and appendicular (126 bones) skeletons.
- Bones are a unique connective tissue with a calcium phosphate matrix.
- Compact (dense) and cancellous (spongy) bone types exist.
- Bone marrow, found in medullary canals, contains red marrow (blood cell production) and yellow marrow (mainly fat).
- Cells, like osteoblasts (bone-forming) and osteoclasts (bone-breaking), maintain bone structure.

Nuclear Medicine Procedures

- PET scanning (18F-NaF, 2-deoxy-2-[18F]fluoro-D-glucose) is used to diagnose, stage and assess metabolic processes in skeletal diseases. (e.g., child abuse, osteomyelitis)
Congenital Skeletal Anomalies
- Congenital skeletal anomalies include osteogenesis imperfecta (OI), achondroplasia, and other skeletal dysplasias.
- Osteogenesis imperfecta, characterized by frequent fractures, is a rare genetic disorder.
- Achondroplasia, the most common inherited skeletal disorder, leads to dwarfism.
Hand and Foot Malformations
- Syndactyly: Failure of fingers or toes to separate, causing webbed appearance.
- Polydactyly: Presence of extra digits.
- Clubfoot (Talipes): Congenital malformation of the foot.
Developmental Dysplasia of the Hip (DDH)
- Malformation of the acetabulum leading to femoral head displacement.
- Presentation: May be unilateral or bilateral, more common in females.
- Risk factors include breech position, first child.
- Diagnosis: Sonography and radiographic measurements.
- Treatment: Early immobilization via casting or splinting. Untreated cases may result in complications like limb length discrepancy
Vertebral Anomalies
- Scoliosis: Lateral curvature of the spine, often convex to the right in the thoracic region, and left in the lumbar region.
- Structural scoliosis includes vertebral rotation.
- Non-structural scoliosis has no vertebral rotation.
- Radiographic assessment involves AP or PA and lateral standing radiographs.
- Treatment includes bracing or casting and/or surgery for severe curves.
- Transitional vertebrae are at thoracic and lumbar vertebral level junctions. Present characteristics of both vertebral levels.
Spina Bifida
- Incomplete closure of vertebral canal, common in lumbosacral area.
- Severity varies from no visible abnormality to paralysis.
- Treatment varies based on extent and severity.
Cranial Anomalies
- Craniosynostosis: Premature closure of cranial sutures resulting in abnormal head shape.
- Anencephaly: Congenital abnormality causing brain and cranial vault failure, often fatal, detected prenatally.
Inflammatory Diseases
- Osteomyelitis: Infection of bone and bone marrow from pathogenic microorganisms.
- Tuberculosis: Chronic inflammatory disease caused by Mycobacterium tuberculosis, often affects hip, knee, and spine.
Arthropathies
- Arthritis, bursitis, tendonitis, and tenosynovitis are joint disorders.
Infectious Arthritis
- Bacteria entering joints cause rapid onset of pain, swelling, and fever.
- Joint effusion, narrowing, recalcification, and sclerosis often occur during healing.
- Staphylococcus aureus and Escherichia coli are common pathogens.
Rheumatoid Arthritis (RA)
- Chronic autoimmune disease affecting synovial tissues and joints.
- Typically, hand and foot joints are primarily affected.
- Radiographic signs include soft tissue swelling, osteoporosis, cortical erosion, and joint space narrowing, leading to joint deformity.
- Diagnosis relies on serologic rheumatoid factor, erythrocyte sedimentation rate, and C-reactive protein levels.
- Treatment includes medications and surgery for severe cases.
Ankylosing Spondylitis
- Progressive form of arthritis, mainly in the spine.
- Common in young men.
- Symptoms include low back pain, morning stiffness, fatigue.
- Radiographic features include bilateral sacroiliac joint narrowing, and spinal ligament calcification.
- Bamboo spine.
- Diagnosis includes HLA-B27 test.
- Treatment includes NSAIDs, anti-inflammatory drugs, exercise, and/or surgery.
Osteoarthritis
- Most common arthritis type frequently associated with aging.
- Affects weight-bearing joints (e.g., hips, knees, ankles).
- Radiographic features - cartilage loss, osteophyte formation, and bone sclerosis.
- Treatment includes medications, exercise, and/or surgery.
Gouty Arthritis
- Metabolic disorder causing uric acid crystal deposits in joints, leading to acute attacks of inflammation and tophi formation.
- Radiographic findings - erosion with overhanging edges.
- Treatment - medication for uric acid excretion or inhibition of its production.
Tendonitis and Tenosynovitis
- Inflammation of tendons and tendon sheaths, potentially with calcifications in chronic cases.
- Treatment includes medications, corticosteroid injections, and possibly surgical intervention.
Bursitis
- Inflammation of bursae (synovial membrane-lined sacs) in common areas like shoulder, elbow, knee and hip.
- Treatment includes aspiration, NSAIDs, corticosteroids, injections, and/or surgical removal.
Ganglion Cyst
- Cystic swelling connected to a tendon sheath, often occurring in the wrist.
- Treatment includes aspiration of the cyst with corticosteroid injections or surgical removal.
Vertebral Column Injuries
- Causes include direct trauma, hyperextension/flexion injuries, osteoporosis, and metastatic destruction.
- Radiographic signs may show interruption of smooth continuous lines, resulting from hyperextension, or hyperflexion injuries.
- Anterospondylolisthesis is anterior slipping of vertebrae commonly occurring at the L5/S1 junction.
Spondylolysis/Spondylolisthesis
- Spondylolysis is a cleft or breakage in the vertebral body, often in the superior arch of the fifth lumbar vertebra.
- Myelography/MRI are often used to evaluate the spinal cord and nerve root for diagnosis.
- Treatment is usually based on severity.
Neoplastic Diseases
- Primary/metastatic tumors.
- Benign tumor types include Osteoma, osteochondroma, simple unicameral bone cyst (UBC), aneurysmal bone cyst (ABC), osteoid osteoma, and osteoblastoma.
- Malignant tumor types include osteosarcoma, Ewing's sarcoma, and chondrosarcoma.
- Diagnosis involves imaging (CT, MRI), biopsy, and staging assessment.
Bone Tumor Markers
- Alkaline phosphatase, osteonectin, osteocalcin, and collagen can be potential bone tumor markers.
- Radiographs, CT scans, and MRI play significant roles to diagnose.
- Enneking's System for bone tumor staging in order to stage, define and classify tumors. This includes assessment of histologic grade, local extent, and presence or absence of metastases.
Common Benign Tumors
- Osteochondroma, osteoma, endochondroma, simple unicameral bone cyst (UBC), aneurysmal bone cyst (ABC), osteoid osteoma, osteoblastoma, giant cell tumor (osteoclastoma).
Malignant Tumors
- Osteosarcoma, Ewing sarcoma, chondrosarcoma (often accompanied by pain), and giant cell tumor (osteoclastoma).
Metastatic Bone Cancer
- Most common malignant tumors are often metastases from carcinomas.
- Commonly affecting individuals over 40.
- Principal signs include pain and pathologic fractures.
- Spine, ribs, sternum, pelvis, skull, and upper ends of femora/humeri are commonly affected.
- Bone scans, rather than X-rays, are more accurate to detect these.
