Radial Nerve Course and Relations
- Descends vertically in the lateral side of the forearm, deep to brachioradialis, 5 cm above the wrist
- Turns backwards to run in the roof of the anatomical snuffbox, where it divides into 5 dorsal digital branches
Radial Nerve Distribution
- Supplies the dorsum of the lateral 2/3 of the hand and the proximal phalanx of the lateral 3 1/2 fingers
Posterior Interosseous Nerve
- Pierces the supinator, turns around the lateral side of the radius to the back of the forearm
- Appears between the supinator and the abductor pollicis longus
- Runs superficial and deep to the extensors, then to the extensor pollicis longus and interosseous membrane
- Ends deep to the extensor retinaculum
Branches in the Forearm
- 2 post-interosseous nerves
- Superficial radial nerve (dorsal digital branches)
- Post-interosseous nerve (motor course)
Effects of Injury
Axilla Injury
- Paralysis of triceps and anconeus leading to loss of elbow extension (elbow drop)
- Paralysis of wrist extensors leading to loss of wrist extension (wrist drop)
- Paralysis of fingers extensors leading to loss of fingers extension (fingers drop)
Spiral Groove Injury
- Motor effects: same as axilla injury, except elbow extension is intact due to long and medial heads of triceps
- Fingers drop due to paralysis of fingers extensors
Superficial Radial Nerve Injury
- Loss of sensation in the back of the lateral 2/3 of the hand and proximal phalanx of lateral 3.5 fingers
