Pulpectomy and Root Canal Treatment (RCT) in Primary Teeth
- Primary molars often have three canals, although some fusion or variations in the number of canals may occur
- Second primary maxillary molars typically have either three (70%) or four (30%) canals
- Mandibular primary molars can have one to three roots, with the double-rooted variant most common
- First mandibular molars usually have two roots, often wider buccolingually and grooved mesiodistally. Most have three (80%) or four (20%) canals
- Second mandibular molars typically have two roots, mesial and distal, and four canals
- Mean root canal lengths differ between different primary molar types and locations (mesiobuccal, mesiolingual, distobuccal, and distolingual)
Primary Tooth Root Canal Physiology and Anomalies
- Roots of primary teeth start resorbing as soon as root length development is complete, impacting apical foramen position
- Accessory canals in primary molars increase the risk of periradicular bone lesions in inflamed teeth, especially in the furcation area
- Root canal anomalies, such as taurodontism (enlarged pulp chamber), and apical displacement of the pulpal floor, and lack of cementoenamel junction constriction may occur, but are not typically requiring modifications to the pulpectomy technique.
Indications and Contraindications for RCT in Primary Teeth
- RCT is indicated when there are clinical signs of irreversible pulpitis or necrosis and minimal/no resorption in the roots
- Contraindications include nonrestorable crowns, pulpal floor perforations, extreme tooth mobility, extensive internal/external root resorption, periradicular radiolucency involving the permanent tooth follicle, and medically compromised patients.
Root Canal Treatment (RCT) Techniques
- RCT in primary teeth can often be completed in one visit
- Access to the pulp chamber is gained after caries removal under local anesthesia and rubber dam isolation
- Canal instrumentation minimizes organic material, taking care not to perforate the root carefully considering the curves of the primary molar roots.
- Canal irrigation is essential for bacterial decontamination using chlorhexidine or sodium hypochlorite (limited to 1%)
Root Canal Filling Materials
- Ideal filling materials should resorb at the same rate as physiological root resorption, be nontoxic, readily resorb if extruded past the apex, antiseptic, easy to insert, and removable if necessary
- Several materials are used. Calcium hydroxide-iodoform mixtures are a common and potentially effective choice due to their high pH and bacteriostatic effect.
- Zinc oxide-eugenol is another option, but there are concerns about potential adverse effects and resorption that might persist after the permanent tooth erupts
