Prescription Guidelines
- Prescriptions must contain the following information:
- Patient's name, age, and gender
- Prescriber's information
- Date of prescription
Generics Act (RA 6675)
- Promotes use of generic names in drug prescriptions.
- Generic name must be written in full, but the salt or chemical form may be abbreviated.
- Generic name must be clearly written immediately after the Rx symbol.
- Brand name may be included in parenthesis below the generic name.
- Pharmaceutical form should also be stated.
Types of Incorrect Prescriptions

Erroneous:
- Brand name (BN) precedes generic name (GN).
- GN is in parenthesis.
- BN is not in parenthesis.

Violative:
- GN is not written.
- GN is not legible, and a legible BN is written.
- BN is indicated with instructions like "no substitution".

Impossible:
- Only GN is written, but not legible.
- GN does not correspond to BN.
- Both GN and BN are not legible.
- Prescribed drug product is not registered with the Philippine FDA.
Rational Medication Use
- Patients receive appropriate medications in individual doses, for adequate time, at lowest cost.
