Postpartum Period
- The postpartum period refers to the first six weeks after delivery.
Rubin's Theory of Maternal Role Adaptation
- The mother goes through three stages during the postpartum period:
- Taking-In
- Taking-Hold
- Letting Go
Taking-In
- This is the first stage after a baby is born. It begins as soon as a woman gives birth.
- Focuses on the mother's physical recovery and meeting her basic needs
- She may be excited but also feel anxious about caring for the baby.
Taking-Hold
- Begins around 2-3 days after delivery.
- The mother becomes more independent and wants to learn.
- The mother feels an urge to care for her baby.
- There is a realization that she is now a mother.
- May still be confused, fearful, and exhausted.
Letting-Go
- This is the final stage.
- The mother accepts her new role and adapts to her new responsibilities.
- This stage begins within a few weeks of delivery and continues until the end of postpartum period.
Vital Signs in the Postpartum Period
- Postpartum hypotension, tachycardia, tachypnea, cold clammy skin are normal.
- A 24-hour high temperature is normal.
- Intermittent increase in temperature may be related to dehydration.
- A decrease of PR to 50-70 bpm after delivery is abnormal.
Striae Gravidarum
- These stretch marks do not disappear but turn silvery-white.
Postpartum Lochia
- Lochia is postpartum vaginal discharge.
- Red lochia on the first postpartum day is normal.
- Pink to brownish lochia on a client 5 days postpartum is also normal.
- Colorless to creamy discharge on a client two weeks after delivery is normal.
- Bright red discharge 5 days after delivery is abnormal.
Fundal Height
- The fundus of the uterus is expected to go down one centimeter per day normally.
Postpartum Voiding
- After vaginal delivery, a woman should void within 6 hours.
Postpartum Interventions
- Assess fetal heart rate (FHR) and maternal blood pressure.
- Assess maternal heart rate and respiratory rate.
- Establish IV access
Lochia Assessment on First Postpartum Day
- Lochia will be dark red, moderate in amount, with some small clots.
Boggy Uterus
- A boggy uterus is one that is soft and flabby.
- If the uterus is boggy four hours after delivery, do not administer Methergin.
Lochia on Second Postpartum Day
- Expected lochia is brown and scant.
Fundal Location on the First Postpartum Day
- The fundus should be located at the level of the umbilicus.
Perineal Care
- Perineal care should be performed from front to back.
Relieving Episiotomy Pain
- Pain relief includes:
- Apply an ice pack
- Take a Sitz bath
- Perform perineal care after each voiding and bowel movement
Postpartum Cesarean Delivery
- Be sure to monitor the client for signs of bleeding.
Postpartum Infection
- Puerperal infection is a postpartum infection.
Postpartum Care
- The nurse should provide education about breastfeeding.
