Study Notes
Post-ROSC BLS Care
- Goal: Maintain adequate oxygenation and ventilation, monitor vital signs, manage airway, ensure blood pressure, and prepare for transport to higher care. Targeted temperature management may apply.
Airway Management
- Clear airway: Position patient properly, use chin lift or jaw thrust as needed.
- Oropharyngeal airway: Consider if patient is unconscious and not breathing adequately.
Breathing Support
- High-flow oxygen: Administer via non-rebreather mask.
- Respiratory monitoring: Monitor rate and depth, adjust ventilations as needed.
Circulation Assessment
- Pulse check: Assess quality and rate.
- Blood pressure management: Monitor blood pressure and address hypotension (e.g., elevate legs, administer fluids if appropriate).
Neurological Assessment
- Level of consciousness: Assess using AVPU scale (Alert, Verbal, Pain, Unresponsive).
- Pupillary and motor function: Check for response.
Targeted Temperature Management (TTM)
- Cooling: Consider cooling to a target range (32°C - 36°C) based on local protocol and patient condition to potentially improve neurological outcomes.
Rapid Transport
- Immediate transport: Transport to appropriate facility with continuous monitoring and support.
Important Considerations
- Continuous monitoring: Regularly assess vital signs (pulse, blood pressure, respiratory rate, level of consciousness).
- Early defibrillation: Initiate if necessary according to ACLS guidelines.
- Communication: Clearly communicate patient condition and interventions to receiving providers.
