Post Incident Evaluation (PIE) Procedure
- Purpose: Review and critique emergency incidents to improve future performance.
- Goal: Educational tool for personnel; identify performance issues without singling out individuals.

Two Types of PIEs:
- Informal: Verbal review of incident, conducted on scene or at station, company level or multiple companies, initiated by lieutenant or chief officer
- Formal: More in-depth review, required for complex incidents (e.g., extended extrications, rescue below/above grade), initiated by deputy or division chief of training, documented with pictures, diagrams, audiotapes
Informal PIE Information
- Conducted at the incident scene, while operations are halted (e.g., hose pickup, equipment transfer)
- Focuses on positive actions and needing improvement
- Reinforces positive events, addresses issues and concerns, before distorted facts
- Does not preclude a formal PIE at a later date
Formal PIE Information
- Information Sources: Gather information from various sources
- Document information before leaving the scene (pictures, sketches, interviews)

- Initial Company Actions: Actions taken by first-in companies (Water supply, RIT team assignments)
- Continued Operations: Actions taken during the ongoing incident
- Outcome: Effects of the actions taken, how this affected the incident
- Lessons Learned: Critique points, observations on how to improve
- Action Plan for Improvement: Future actions to improve responses to similar incidents
