Zosyn Dosage
- Administer IV for most infections at a dosage of 3.375 g every 6 hours.
- For nosocomial pneumonia, increase dosage to 4.5 g every 6 hours.
Zosyn Classification
- Therapeutic classification: anti-infective.
- Pharmacologic classification: extended spectrum penicillins.
Zosyn Actions
- Causes death of susceptible bacteria by binding to bacterial cell wall membranes, leading to cell death.
- Piperacillin has an extended spectrum compared to other penicillins.
- Tazobactam inhibits the enzyme beta-lactamase, which can inactivate penicillins.
- Effective against piperacillin-resistant, beta-lactamase-producing organisms including:
- Bacteroides fragilis

Zosyn Uses

Zosyn Contraindications
- Contraindicated in patients with hypersensitivity to penicillins, beta-lactams, or cephalosporins.
- Use with caution in patients with renal impairment and sodium restriction.
Zosyn Side Effects
- Common side effects:
- Diarrhea
- Rashes
- Pain or phlebitis at the IV site.
- Less common but serious side effects may include:
- Anaphylaxis

- Clostridioides difficile-associated diarrhea (CDAD)
- Stevens-Johnson syndrome

Zosyn Nursing Implications
- Evaluate renal and hepatic function, CBC, serum potassium levels, and bleeding times.
- Collect specimen for culture and sensitivity before therapy.
- Assess patient's history for reactions to penicillins.
- Monitor for signs and symptoms of infection and anaphylaxis.
- Regularly check bowel function and skin reactions.
- Be vigilant for signs of DRESS: fever, rash, swelling, lymphadenopathy.
- Administer infusion over 30 minutes.
Zosyn Patient Teaching
- Advise patients to report rash or any skin reactions immediately.
- Instruct to notify healthcare provider if experiencing fever, diarrhea, or blood/pus/mucus in stool.
