2

What is the overall goal of anti-platelet therapy?

To inhibit platelet activation

What is the primary benefit of anti-platelet therapy in patients with prosthetic heart valves?

Inhibition of embolization

In which type of patients is anti-platelet therapy particularly important?

Those with prosthetic heart valves

What is the consequence of inadequate anti-platelet therapy in patients with prosthetic heart valves?

Higher risk of embolization

What is the primary goal of anti-platelet therapy in patients with prosthetic heart valves?

To prevent platelet aggregation

Why is anti-platelet therapy important in patients with prosthetic heart valves?

To prevent the formation of blood clots

What is the classification of Dipyridamole?

Anti-platelet

What is the purpose of Dipyridamole?

To prevent blood clots

Which of the following is NOT a characteristic of Dipyridamole?

Increases platelet count

What is the effect of Dipyridamole on platelet activity?

Inhibits platelet activation

What type of medication is Dipyridamole?

Anti-platelet

What is the optimal dose of the medication?

80mg

What is the effect of high doses of the medication on COX-2?

It inhibits COX-2

Why are higher doses of the medication potentially less efficacious?

Because they inhibit COX-2

What is the effect of higher doses of the medication on toxicity?

It increases toxicity, especially bleeding

What is prostacyclin responsible for?

Inhibiting platelet aggregation

Decrease in WBC

What is the dose of Ticlopidine?

250 mg BD

Which of the following is NOT an adverse effect of Ticlopidine?

Headache

ADP antagonists

Which of the following is a safer option than Ticlopidine?

Clopidogrel

Study Notes

Anti-Platelet Drugs

  • To give anti-platelet drugs, four mechanisms are targeted:
    • Acetylsalicylic acid (aspirin) as an inhibitor of Thromboxane A2
    • P2Y12 antagonists
    • GPIIb/IIIa antagonists
    • Dipyridamole

Mechanism of Acetylsalicylic Acid (Aspirin)

  • Works as an inhibitor of Thromboxane A2
  • At high doses (1g/d), also inhibits COX-2, responsible for synthesizing prostacyclin
  • High doses are potentially less efficacious and increase toxicity, especially bleeding
  • In patients with prosthetic valves, it inhibits embolization

Comparing Clopidogrel and Ticlopidine

  • Clopidogrel is safer than Ticlopidine
  • Ticlopidine has adverse effects, including:
    • Diarrhea
    • Vomiting
    • Abdominal pain
    • Decrease in WBC

Dipyridamole

  • Has two effects: anti-platelet and maximal effective dose of 50-320 mg (optimal dose = 80mg)

This quiz covers the mechanisms of action of anti-platelet drugs, including acetylsalicylic acid and P2Y12 antagonists.
