    Study Notes

    Anti-Platelet Drugs

    • To give anti-platelet drugs, four mechanisms are targeted:
      • Acetylsalicylic acid (aspirin) as an inhibitor of Thromboxane A2
      • P2Y12 antagonists
      • GPIIb/IIIa antagonists
      • Dipyridamole

    Mechanism of Acetylsalicylic Acid (Aspirin)

    • Works as an inhibitor of Thromboxane A2
    • At high doses (1g/d), also inhibits COX-2, responsible for synthesizing prostacyclin
    • High doses are potentially less efficacious and increase toxicity, especially bleeding
    • In patients with prosthetic valves, it inhibits embolization

    Comparing Clopidogrel and Ticlopidine

    • Clopidogrel is safer than Ticlopidine
    • Ticlopidine has adverse effects, including:
      • Diarrhea
      • Vomiting
      • Abdominal pain
      • Decrease in WBC

    Dipyridamole

    • Has two effects: anti-platelet and maximal effective dose of 50-320 mg (optimal dose = 80mg)
