Histamine-2 Receptor Antagonists
- Famotidine is the primary medication classified as a histamine-2 receptor antagonist.
- Other medications such as lansoprazole and esomeprazole are proton pump inhibitors (PPIs) and should not be confused with H2 antagonists.
- Metronidazole is classified as an antibiotic and is also unrelated to H2 antagonists.
Dobutamine and Cardiac Output
- Dobutamine is primarily used to increase cardiac output.
- It is indicated for clients with acute heart failure and those undergoing cardiopulmonary bypass surgery.
- Catecholamines like epinephrine hydrochloride are used for sinus bradycardia and acute hypotension, but not for hypertension due to their potential to elevate blood pressure.
Antimicrobials for Burns
- Tetracycline is not commonly used in the treatment of burns.
- Silver sulfadiazine (Silvadene), mafenide (Sulfamylon), and silver nitrate (AgNO3) 0.5% solution are the three main antimicrobials prescribed for burn management.
Management of Unrelieved Pain
- In cases of unrelieved heel pain after analgesic administration, the appropriate nursing intervention is to contact the physician.
- The nurse cannot administer a repeat dose of analgesic without a physician's order.
- Other interventions like massaging the ankle or applying moist heat are inappropriate as they could lead to complications such as deep vein thrombosis.
Administration of Adenosine
- Adenosine should be administered via IV push as quickly as possible for treating supraventricular tachycardia (SVT).
- Due to its extremely short half-life of a few seconds, it must be followed by a rapid flush to ensure effectiveness.
- Slower administration or dilution in saline results in loss of efficacy and is not recommended.
