- CYP2D6 is medically necessary to guide treatment and/or dosing for individuals for whom initial treatment is planned with amitriptyline or nortriptyline for depressive disorders or tetrabenazine doses greater than 50 mg/day, or re-initiation of therapy with doses greater than 50 mg/day2.
