Study Notes

Periodontal Ligament

  • Attaches teeth to surrounding alveolar bone by way of cementum
  • Covers the root of the tooth and connects with gingiva tissue
  • Occupies the periodontal space and is composed of fibers, cells, and intercellular substance
  • Develops from dental follicular tissue that surrounds the tooth

Composition

  • Highly cellular, containing fibroblasts, vascular, neural, bone, and cemental cells
  • Interstitial spaces contain blood vessels, nerve trunks, and communicate freely with vessels and nerves at the apex of the roots and alveolar bone

Radiographic Appearance

  • Appears as a radiolucent area (dark) on radiographs, between the radiopaque (light) lamina dura of the alveolar bone proper and the radiopaque (light) cementum

Cells

  • Fibroblast is the most common cell in the PDL
  • Also contains cells not found in other connective tissues, such as cementoblasts, osteoblasts, osteoclasts, odontoclasts, and epithelial rests of Malassez

Organization

  • Two groups of principal fibers: gingival group and dentoalveolar/alveolodental group
  • Gingival group is located around the necks of the teeth, while dentoalveolar/alveolodental group surrounds the roots of the teeth
  • Four groups of gingival fibers: dentogingival, alveologingival, circular, and dentoperiosteal fibers

Interstitial Space

  • Contains a network of blood vessels, nerves, and lymphatics that maintain the vitality of the periodontal ligament
  • Supports the dense collagen fiber bundles and allows for constant stretching and contraction of the fiber bundles during mastication

Functions

  • Maintains the gingiva in proper relationship to the teeth
  • Transmits occlusal forces from the teeth to the bone, allowing for a small amount of movement and acting as a shock absorber
  • Serves as the periosteum for the cementum and alveolar bone
  • Cells in the PDL participate in the formation and resorption of the hard tissues of the periodontium
  • Provides nutrition for the cells of the ligament and surrounding cells of the cementum and alveolar bone
  • Has a nerve supply that provides a proprioceptive mechanism, allowing for the sensation of even the most delicate forces applied to the teeth and any displacement of the teeth
