What is the primary objective of root planing?
- To remove exclusively extrinsic stains from the enamel surface.
- To remove only supragingival calculus.
- To smooth root surfaces by removing dental plaque, calculus, and affected cementum or dentine. (correct)
- To perform a gross scale of built-up plaque.

What is the key characteristic that differentiates 'fine scale' from 'gross scale'?
What is the key characteristic that differentiates 'fine scale' from 'gross scale'?
- Gross scale is appropriate for patients with minimal bleeding sites.
- Gross scale is only used for patients who have had regular professional cleaning.
- Fine scale is more suited for removing remaining build-up and stains, especially when bone involvement and probing depths are minimal. (correct)
- Fine scale is used exclusively on patients with heavy supragingival deposits.

Study Notes
- The session focuses on understanding the principles of periodontal instrumentation.
- Students should become familiar with the terminology and objectives of various periodontal instrumentation techniques and their anatomical locations.
Periodontal Instrumentation Terminology
- Scaling is the removal of dental plaque and calculus.
- Root planing involves removing dental plaque, calculus, and smoothing root surfaces. It includes removing dentine or cementum
Curettage
- Curettage removes the soft tissue lining of the periodontal pocket with a curette, leaving only a gingival connective tissue lining. Gingival curettage concept was designed to promote new connective tissue attachment by removing pocket lining and junctional epithelium.
Sub and Supra Gingival Scaling
- Subgingival scaling addresses areas below the gumline.
- Supragingival scaling deals with areas above the gumline.
Fine Scale
- Is used to remove remaining build-up and stains. It is suited for patients with heavy deposits and minimal bone involvement, with probing depths less than 3mm.
Gross Scale
- Gross scale is common for patients who have not had their teeth professionally cleaned for a long time. It allows for evaluation and facilitates home cleaning.
Non-Surgical Periodontal Instrumentation
- Prophylaxis is for patients with healthy gum tissue to remove plaque, calculus, and extrinsic stains to maintain a healthy mouth, with <10% bleeding sites
Supra Professional Mechanical Plaque Removal (PMPR)
- Is a first-step therapy. This includes personalized oral hygiene advice, patient education on risk factors, removal of stain and plaque and cleaning the gingival crevice. This is done by hand or power driven devices
Root Surface Debridement and Sub-Gingival Instrumentation
- Root surface debridement and sub-gingival PMPR is a second step therapy.
Supportive Periodontal Therapy/Care
- Supportive periodontal therapy reduces the probability of re-infection, controls the disease ensures no pain, prevent teeth mobility/infection and prevent related oral diseases.
Key Considerations
- Familiarity with instrumentation terms.
- The appropriateness of instrumentation.
- Challenges for clinicians and patients.
