    Study Notes

    Neonatal Care

    • The Neonatal period is critical, and prompt care can reduce childhood mortality in support of the Millennium Developmental Goal #4.

    Airway

    • Asphyxiation is a leading cause of death in neonates, and it can be caused by umbilical cord clamping, leading to hypoxia, hypercapnia, acidosis, CNS depression, and potentially death.
    • If the umbilical cord is not cut, reverse circulation can lead to Rh Incompatibility.
    • To prevent asphyxiation, neonates should breathe after birth, and crying stimulates breathing.
    • A crying baby is a breathing baby.
    • Suction with a bulb syringe PRN is necessary if there's nasal obstruction or meconium-stained amniotic fluid to prevent aspiration.
    • Avoid using wall suction as it may cause deep suctioning, leading to hypoxia, vagal stimulation, and arrhythmias.

    Body Temperature

    • Physiologic heat loss occurs after birth, and extreme hypothermia can cause acidosis.
    • Extreme hypothermia can lead to shivering, increased oxygen consumption, decreased oxygen, and increased CO2, resulting in acidosis.
    • Babies are prone to cold stress due to extreme hypothermia, which can cause acidosis.
    • Management of hypothermia includes:
      • Drying the baby immediately after birth to reduce heat loss by evaporation.
      • Placing the baby on top of the mother's body, skin to skin (conduction), also known as "Unang Yakap".
      • Using heating devices, such as gooseneck lamps, floor lamps, or radiant warmers, with caution to avoid burns.
      • Avoiding cold drafts and postponing the bath until the temperature is stable (6 hours).
      • Using warm water during bathing.
