Ask the patient to bring all medications.
- Substance Use: Inquire about smoking, alcohol, and drug use.
- Chronic Diseases: Detail major illnesses like hypertension, diabetes, asthma, heart failure, angina, or stroke, including onset age, severity, organ involvement, medications (and recent changes), last evaluations (e.g., stress tests), and referring physician/clinic.
- Minor Illnesses: Record recent minor illnesses like bronchitis.
- Hospitalizations: Note any hospitalizations, reasons, and durations.
- Blood Transfusions: Document any blood transfusions.
- Allergies: Record any allergies, including specific reactions (e.g., rash, nausea, anaphylaxis). Ask about allergies to food, insects, and environmental factors.
- Past Surgical History: Detail procedures, indications, outcomes, surgeons, hospitals, and complications (e.g., anesthetic complications, difficult intubations).
