Passport Appointment Information
- Appointment is on October 24, 2024 from 2:00 PM to 3:00 PM at SM North Edsa - Temporary Off-site Passport Service
- Applicant is Corazon Valeros Espeso
- Applicant must arrive 15-30 minutes before the appointment
- Applicant must bring printed application form and e-receipt
- Applicant must bring both original and photocopies of all required documents
- Passport processing fee is non-refundable and non-transferable
Passport Application Process
- Passport application is with courier service
- Payment was made through Paymaya and GCash
- Payment of ₱950.00 for passport fee and ₱250.00 for PRF fee
- Applicant must check in at the verification section
- Applicant must present application form, ID, and e-receipt to verify appointment
- Applicant must present verified application form and requirements to the processor
- Applicant must proceed to the data capturing/encoding section
- Applicant must review all information entered and sign the electronic confirmation page
Additional Passport Information
