Flashcards

Surface Epithelial Ovarian Tumors

Most common type of ovarian tumor, accounting for 70% of cases. These tumors arise from the coelomic epithelium, which is the same embryonic tissue that forms the fallopian tube lining and the endometrium.

Serous Tumor

A type of surface epithelial tumor characterized by watery fluid. Can be benign, borderline, or malignant.

Mucinous Tumor

A type of surface epithelial tumor characterized by thick mucous fluid. Can be benign, borderline, or malignant.

Cystadenoma

A benign tumor composed of a single cyst with a simple lining. Commonly found in premenopausal women.

Cystadenocarcinoma

A malignant tumor composed of complex cysts with a thick lining. Commonly found in postmenopausal women.

Borderline Tumor

A tumor with features between benign and malignant. Has a better prognosis than malignant tumors, but still carries metastatic potential.

Endometrioid Tumor

A surface epithelial tumor composed of endometrial-like glands. Usually malignant.

Study Notes

Ovarian Tumors: Basic Principles

  • Ovaries consist of three cell types: surface epithelium, germ cells, and sex cord-stromal cells
  • Ovarian tumors can originate from any of these cell types or through metastasis
  • Surface epithelial tumors are the most common type (70%)

Surface Epithelial Tumors

  • These tumors arise from the coelomic epithelium lining the ovary
  • This epithelium also develops the lining of the fallopian tubes, endometrium, and endocervix
  • Common subtypes are serous and mucinous, typically cystic
    • Serous tumors contain watery fluid
    • Mucinous tumors contain mucus-like fluid
  • These tumors can be benign, borderline, or malignant
    • Benign (cystadenomas): characterized by a single cyst with a simple lining; typically affect premenopausal women (aged 30-40)
    • Malignant (cystadenocarcinomas): involve complex cysts with thick, irregular lining; common in postmenopausal women (aged 60-70)
    • Borderline tumors have characteristics between benign and malignant; have a better prognosis than malignant tumors but still have metastatic potential
  • BRCA1 mutation carriers have a higher risk of serous ovarian and fallopian tube carcinoma
    • Prophylactic salpingo-oophorectomy (removal of fallopian tubes and ovaries) and prophylactic mastectomy (breast removal) may be considered for these carriers
  • Other less common subtypes include endometrioid and Brenner tumors
    • Endometrioid tumors are typically malignant and resemble endometrial tissue
    • Brenner tumors are usually benign and resemble bladder tissue

Additional Information

  • Prognosis for surface epithelial carcinoma is generally poor, as it is one of the worst prognoses in female genital tract cancers
  • Epithelial carcinomas, in general, tend to spread locally, especially to the peritoneum
  • CA-125 is a serum marker useful to monitor treatment response and screen for recurrence of ovarian cancer
