- Typically first molars are examined, but if missing, the first fully erupted molar distal to the second premolar is used
- In the anterior region, facial surfaces of the maxillary right and mandibular left central incisors are scored
- If a central incisor is missing, the adjacent central incisor is scored
- Scoring includes the facial or lingual surfaces of selected teeth and proximal surfaces up to the contact areas
- At least two of the six possible surfaces must be examined to calculate an individual score
- Oral debris includes soft foreign matter like dental biofilm, materia alba, and food debris
- A probe or explorer estimates the surface area covered by debris
Simplified Debris Index (DI-S) Criteria
- Score 0: No debris or stain present
- Score 1: Soft debris covering ≤1/3 of the tooth surface or extrinsic stains without debris
- Score 2: Soft debris covering >1/3 but ≤2/3 of the exposed tooth surface
- Score 3: Soft debris covering >2/3 of the exposed tooth surface
- Calculus is evaluated by using an explorer to estimate the surface area covered by supragingival deposits and by exploring/probing for subgingival deposits
- Scoring is based on location and tooth surface area covered
