- An organized group of activities used to identify and address nutritional needs.
- Parallels the Nursing Care Process, focusing on nutrition-related concerns.
- Consists of four organized steps: Assessment, Diagnosis, Intervention, and Monitoring & Evaluation.
Important Terms
- Nutrition Care Process (NCP): A systematic approach for nutrition and dietetics professionals to address nutrition-related problems and provide quality care.
- Nutrition Assessment: A systematic process to obtain, verify, and interpret data about a patient's nutritional status. This step uses ABCDs of assessment: Anthropometric, Biochemical, Clinical, and Dietary.
- Nutrition Diagnosis: Identification and labeling of nutritional problems present, likely to occur, or potentially occurring.
- Nutrition Intervention: Purposeful planned actions to change the nutrition-related factor, condition, behavior, risk, or aspect of the patient's health status to meet nutritional needs.
- Nutrition Monitoring and Evaluation: Determining if nutrition care goals have been met through continuous monitoring using relevant and specific markers or indicators against recognized science-based standards. This also checks and determines if any reassessment is necessary.
ABCDs of Nutrition Assessment
- Anthropometric: Measurements of body size and shape (e.g., height, weight, BMI, circumference). Indicators of protein and energy stores.
- Biochemical: Routine lab tests (e.g., serum electrolytes, BUN, creatinine, blood glucose, lipid profile, liver enzymes, CBC, cholesterol). Help identify malnourishment.
- Clinical: Head-to-toe assessment. Physical signs of over or undernutrition easily identified during an examination.
- Dietary: Involves asking subjective questions about the patient's daily dietary habits (e.g., food frequency, 24-hour recall, food diary, interview).
Nutrition Diagnosis/PES Statement
- A concise summary of the nutritional problem, cause, and contributing risk factors.
- Includes problem statement (diagnostic label), etiology or contributing risk factors, and signs/symptoms.
- This step details nutritional problems that dietetics professionals address.
Nutrition Intervention
- Purposeful actions planned to change a specific nutrition-related issue such as behavior, risks, conditions, or aspects of a patient's health.
- This involves selecting, planning, and implementing appropriate actions to meet the patients/clients/groups' nutrition needs.
- The selection of nutrition interventions is driven by the nutrition diagnosis. This step involves using certain methods/actions to achieve the objectives.
Nutrition Monitoring and Evaluation
- Continuous monitoring to evaluate the effectiveness of the intervention and determines if any reassessment is needed.
- Uses outcome indicators (markers) that are relevant to the patient's defined needs, nutrition diagnosis, goals, and disease conditions.
Clinical Nutrition Therapy
- The direct treatment of a patient's nutritional needs using various approaches within the NCP.
Primary Role of Registered Nutritionist-Dietitians
- Provide and implement the Nutrition Care Process.
- Improve health, promote faster recovery, and prevent malnutrition.
- Provide expert guidance and counselling and service appropriate meals.
- Work within a interdisciplinary team.
Nutrition Care Process in Hospitalized Patients (In-Patients)
- A systematic process for implementing nutrition care for hospitalized patients covering steps like screening, assessment, diagnosis, intervention, and monitoring & evaluation. Shows the detailed flow of the process.
The Multidisciplinary Team in Nutrition Care
- Attending Physician: Refers cases, conforms to the NCP, and leads the team.
- Registered Nutritionist-Dietitian: Develop nutrition care plan, implements it, monitors, evaluates progress, and participates in case conferences.
- Registered Nurse: Completes nutrition screenings, carries out care instructions, updates diet lists, documents changes, and communicates with RND.
