The Nursing Process
- A systematic, rational method for planning and providing nursing care

Purpose:
- Identify a client's health status
- Identify actual or potential health care problems or needs
- Establish plans to meet the identified needs
- Deliver specific nursing interventions to meet those needs
Components of the Nursing Process
- Assessing: Collecting, organizing, validating, and documenting data.
- Diagnosing: Analyzing and synthesizing data to identify client strengths, health problems, and develop a list of nursing and collaborative problems
- Planning: Determining how to prevent, resolve, or reduce identified client problems, supporting client strengths, implementing nursing interventions in an organized, individualized, and goal-directed manner.
- Implementing: Carrying out and documenting planned nursing interventions.
- Evaluating: Measuring the degree to which goals/outcomes have been achieved, identifying factors influencing goal achievement, and determining whether to continue, modify, or terminate the plan of care

Assessment
- A systematic and continuous collection, organization, validation, and documentation of data
- All phases of the nursing process depend on accurate and complete data collection.
Types of Assessment
- Initial Nursing Assessment: Performed within a specified time period, establishes a complete database.
- Problem-Focused Assessment: Ongoing process integrated with care, determines the status of a specific problem.
- Emergency Assessment: Performed during a physiologic or psychological crisis, identifies life-threatening problems.
- Time-Lapsed Assessment: Occurring several months after the initial assessment, compares current status to baseline.
Process of Assessment
- Collecting data: Gathering information about a client's health status, must be systematic and continuous to prevent the omission of significant data.
- Organizing data: Systematically preparing the data collected for analysis, often using a structured format.
- Validating data: Verifying or confirming the accuracy and reliability of data collected, often by comparing with other sources or repeating measurements.
- Documenting data: Recording the collected data accurately, concisely, and in a timely manner in the client's record, using approved terminology and formats.
Collecting Data
- The process of gathering information about a client's health status.
- Should include past history as well as current problems.
- Database: All information about the client, including health history, physical assessment and examination, results of diagnostic and laboratory tests.
Subjective Data
- Symptoms: Covert data apparent only to the person affected, described only by the person.
- Includes sensations, feelings, values, beliefs, attitudes, and personal health status.
Objective Data
- Signs: Overt data detectable by an observer, measurable or tested against an accepted standard.
- Can be seen, heard, felt, or smelled.
- Obtained through observation or physical examination.
Sources of Data
- Primary Source: The client
- Secondary Sources: All other sources of data such as family members, spouses, support people, primary care providers. Should be validated if possible.
Methods of Data Collection
- Observing: Gathering data using the senses (sight, smell, hearing, touch).

Interviewing: Planned communication or conversation with a purpose.
- Used to identify problems of mutual concern, evaluate change, teach, provide support, provide counseling or therapy.
Approaches to Interviewing

Directive Approach: Highly structured and elicits specific information.
- The nurse establishes purpose and controls the interview.
- Non-Directive Approach: Rapport building approach where the client controls the subject matter, purpose, and pacing.
Types of Interview Questions
- Closed Questions: Restrictive, yes/no, factual, less effort and information from the client.
- Open-Ended Questions: Specify a broad topic, invite longer answers, get more information from the client, useful for changing topics and eliciting attitudes.
- Neutral Questions: Allow the client to answer without direction or pressure.
- Leading Questions: Direct the client's answers, used in a directive interview approach.
