NIS Login

NIS Enrollment Form

- Upload a recent image in .jpg, .jpeg, or .png format, less than 2MB.
- Enter their full name, gender, Aadhaar number, date of birth, father's name, mother's name, mobile number, email address, and nationality.
- Select their institute type (college, school, inter college, or sainik school).
- Enter their pincode, wing, and educational qualification.
- Choose their school/college and group name.
- Provide their address information, including house number, building name, pincode, area/locality, and city.
- Enter medical details, including identification mark, blood group, and medical complaint (if any).
- Provide Next of Kin (NOK) information, including name, relationship with the user, and contact number.
- Enter the NOK's address, including house number, building name, pincode, area/locality, and city.
NIS Document Upload and Submission
