Study Notes
Multiple Gestation (Twin Pregnancy)
- Multiple gestation is a pregnancy with more than one fetus, occurring in 1-2% of pregnancies.
- Two-thirds of twin pregnancies are dizygotic, and one-third are monozygotic.
Types and Classification of Twins
-
Dizygotic (Non-identical) Twins
- Result from the fertilization of two separate eggs.
- Each twin has its own placenta and amniotic sac (dichorionic diamniotic).
- Fetuses can be different sexes.
-
Monozygotic (Identical) Twins
- Result from the splitting of a single fertilized egg.
- Can share the same placenta (monochorionic), amniotic sac (monoamniotic), or even fetal organs (conjoined twins).
- Chorionicity and amnionicity depend on the timing of the split:
- Dichorionic diamniotic: Splitting within the first 3 days after fertilization.
- Monochorionic diamniotic: Splitting between days 4-8 after fertilization.
- Monochorionic monoamniotic: Splitting between days 8-12 after fertilization.
- Conjoined: Splitting after day 13.
Predisposing Factors
-
Dizygotic Twins
- Ethnic group: Higher in African populations, lower in Asian populations.
- Maternal age: Increases with age, reaching 2% at age 35.
- Parity: Increases after four pregnancies.
- Method of conception: 20% with ovulation induction.
- Family history
-
Monozygotic Twins
- No identifiable risk factors.
Complications of Twin Pregnancy
- Prematurity: Half of twins are delivered preterm (before 37 weeks).
-
Perinatal mortality: Six times higher than singleton pregnancies.
- Caused mainly by prematurity and twin-to-twin transfusion syndrome (TTTS) in monochorionic twins.
-
Intrauterine growth restriction (IUGR):
- Higher risk in twins, especially monochorionic twins.
- Can affect one or both fetuses.
-
Fetal abnormalities:
- Prevalence of structural abnormalities is higher in twins, especially monochorionic twins.
- Chromosomal defects are more common in monozygotic twins.
-
Death of one fetus:
- Can be associated with poor outcomes for the co-twin.
- Higher risk of death or handicap for co-twin in monochorionic twins due to placental vascular anastomoses.
Management of Twin Pregnancy
-
Antepartum:
- Gestational diabetes: Routine screening due to increased diabetogenic hormones.
- Anaemia: Increased prevalence due to increased plasma volume and fetal demand for iron and folic acid.
- ‘Minor’ symptoms: More common and severe due to increased uterine size and hormones.
- Antepartum haemorrhage: Increased prevalence due to larger placental area and other complications (hypertensive disease).
- Thromboembolic disease: Increased risk due to physiological changes and vascular compression.
-
Intrapartum:
-
Mode of delivery:
- Vaginal delivery possible if the first twin presents vertex and there are no complications.
- Elective Caesarean section is preferred by some obstetricians.
- Decision based on obstetric history, presentation, complications, and maternal preference.
-
Mode of delivery:
Tests for Multiple Gestations
- Ultrasound: To diagnose multiple gestation and determine chorionicity and amnionicity.
- Maternal serum biochemistry: Less effective for screening for Trisomy 21 in multiple gestations.
- Nuchal translucency: Ultrasound measurement at 10-14 weeks to screen for chromosomal defects in each fetus.
- Invasive prenatal diagnosis: Amniocentesis or chorionic villus sampling performed in specialized centers for accurate and safe fetal sampling.
