  • A patient care model is a method of organizing and delivering nursing care to achieve desired patient outcomes.

Types of Patient Care Models

  • Case method or total patient care: one nurse is assigned to one patient for the delivery of total care.
  • Functional method: nursing tasks are divided among nurses.
  • Team or modular method: a group of nurses work together to provide care.
  • Primary method: one nurse is responsible for a group of patients.
  • Partnership model: two nurses, a registered nurse and a partner, work together to provide care.
  • Case management: a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs.

Case Management

  • Components: assessment, planning, implementation, evaluation, and interaction.
  • RN case manager assumes a planning and evaluative role, not responsible for direct care duties.
  • Focuses on individual patients and supervises care across the continuum.

Advantages and Disadvantages of Case Management
