Patient Care Models
- A patient care model is a method of organizing and delivering nursing care to achieve desired patient outcomes.
Types of Patient Care Models
- Case method or total patient care: one nurse is assigned to one patient for the delivery of total care.
- Functional method: nursing tasks are divided among nurses.
- Team or modular method: a group of nurses work together to provide care.
- Primary method: one nurse is responsible for a group of patients.
- Partnership model: two nurses, a registered nurse and a partner, work together to provide care.
- Case management: a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs.
Case Management
- Components: assessment, planning, implementation, evaluation, and interaction.
- RN case manager assumes a planning and evaluative role, not responsible for direct care duties.
- Focuses on individual patients and supervises care across the continuum.
Advantages and Disadvantages of Case Management
- Advantages: can be cost-effective, satisfying for high-risk patients.
- Disadvantages: RN may have difficulty delegating to partner, consistent partnership difficult to maintain due to varied schedules.
Factors Affecting Selection of Patient Care Model
- Availability of registered professional nurses.
- Philosophy of the nursing services division.
- Patient characteristics (patient acuity).

- Organizational support (top-level health facility administration).

Case Method or Total Patient Care
- Advantages: possible for one nurse to be assigned to more than one patient, nurse can meet objectives, assigning patients is simple, direct and clear lines of responsibility and accountability.
- Disadvantages: patient has a different nursing approach each shift, often resulting in confusion, requires highly skilled nursing personnel, can be very expensive.
Effective Patient Care Models in Different Situations
- Case method: effective in special circumstances, where single care is needed (isolation, intensive care units, home health agencies).
- Primary method: effective in Maternity Hospital, where care plans include delivery care, vaccination plan, consultation, and referral to health care center and follow-up at home.
- Case method: effective in Surgical Department, where a nurse provides 24-hour care for a patient, including preparation for operation and post-operative care.
