- Medical Protocol 42 covers adult drug overdose, emergencies related to alcohol, and poisoning cases.
- For poison control, the number to call is (800) 222-1222.
- The protocol is for Basic EMT, Intermediate EMT, & Paramedic levels
Initial Medical Care
- Obtain a blood glucose reading.
- If available, use a cardiac monitor for a 12-lead ECG.
Suspected Narcotic Overdose
- If a narcotic or synthetic narcotic overdose is suspected and the respiratory rate is less than 12, consider administering Naloxone (Narcan) 2mg IN.
- Administer Naloxone (Narcan) 2mg IV/IO/IM/IN, considering restraints prior to administration.
- Naloxone may be repeated every 5 minutes as necessary, up to a total of 6mg.
Altered Consciousness or Low Blood Sugar
- If there is an altered level of consciousness and the cause is unknown, OR if blood sugar level is less than 60, take the following steps.
- If the patient is awake and can tolerate PO (oral) intake, administer small amounts of oral glucose.
- Alternatively, consider Glucagon 1mg IN if available.
- If blood sugar level is less than 60, administer 50% Dextrose 50ml IV/IO OR Glucagon 1mg IM/IN if an IV cannot be established.
- Transport the patient after these interventions.
- For pediatric patients, refer to the Pediatric Altered Level of Consciousness/BRUE Protocol #71.
- This protocol was revised on 12/01/21 and became effective on 05/01/98.
