  • Introduce yourself and obtain consent (verbal permission) before taking a patient's history
  • Follow a structured approach: History of Presenting Illness (HPI), Analysis of patient complaints, Systemic Review, Past Medical History, and Family History

History of Presenting Illness (HPI)

  • Identify and analyze the patient's chief complaint and its duration
  • Document the history of present illness, including systemic review of the affected system

Systemic Review

  • Ask about symptoms related to other systems, including:
    • Urinary system: blood in urine, loin or suprapubic pain, burning micturition, etc.
    • Musculoskeletal system: joint or muscle pain, skin rashes or ulcers, etc.

Past Medical History

  • Inquire about:
    • History of similar conditions
