Low Ulnar Nerve Palsy ("Claw Hand")
- Nerve Involved: Ulnar nerve
- Site: Low or distal
- Digits Affected: Digits 4 and 5 are most affected
- Muscles Paralyzed:
- Lumbricals of digits 4-5
- Interossei
- Adductor pollicis
- Presentation with open palm at rest:
- Metacarpophalangeal (MCP) joints 4-5 are hyperextended
- Interphalangeal (IP) joints 4-5 are flexed
- Interphalangeal (IP) joint 1 (thumb) is hyperextended when pinching
- Presentation when making a fist: Normal
- Additional clinical signs:
- Hypothenar flattening or wasting
- Weak pinch
- Loss of grip strength
- Lack of finger abduction and adduction
