Lost or Stolen Items
- Lost or stolen inventory items are replaced after submitting a requisition through proper channels to the Bureau Commander.
- An F-225 letterhead report is required any time an inventory item is lost or stolen.
- If an inventory item valued at $25 or more is lost or stolen, a report must be filed with the local Police Department and the report number included in the F-225.
- Battalion/Section Commanders must conduct a thorough investigation of any property loss and report findings, including negligence or carelessness of personnel.
- Radios loaned from allied agencies must be promptly reported to the Metro Battalion Chief by the Administrative Battalion Chief if lost or stolen.
- A report on the loss must be submitted by the Battalion/Section Commander, either as an addition to the original report or as a separate F-225 letterhead report.
Turn-ins and Supplies
- Surplus expendable items must be turned into S&M when no longer of value; requires a SRS requisition with current stock codes for the return.

Inventory Item Procedures

Exchange or Repair of Items
