Classification
- Antidysrhythmic: Affects heart rhythm by slowing spontaneous cardiac contractions and ventricular electrical activity
- Anesthetic: Interrupts nerve impulse transmission, providing localized anesthesia
Medical Uses
- Alternative treatment to Amiodarone for cardiac arrest due to Ventricular Tachycardia (Vtach) or Ventricular Fibrillation (Vfib) with wide QT interval
- Acute management of ventricular dysrhythmias
- Utilized as a procedural anesthetic, including in intubation and intraosseous (IO) access
Administration Routes
- Administered via intravenous (IV), intraosseous (IO), and subcutaneous (SubQ) methods
Contraindications
- Not to be used in patients with 2nd or 3rd degree AV blocks, Stokes-Adams syndrome, or in the prophylactic treatment of acute myocardial infarction (AMI)
- Avoid in cases of wide-complex ventricular escape beats accompanied by bradycardia
Adverse Effects

- Other serious effects: slurred speech, respiratory arrest, hypotension, bradycardia, various dysrhythmias, cardiac arrest, AV block, nausea, and vomiting
Special Considerations
- Dose reduction by 50% recommended for patients over 70 years old or those with liver or renal disease, heart failure, or in shock
- Excessive doses may lead to coma or death
