Completing Field Reporting Forms

  • The Accident/Loss Report is completed when agency property is lost/damaged, or a member damages another's property, including agency vehicle traffic crashes

    • The form has seven sections, all of which the member must complete.
  • Use the Additional Stolen Items Inventory List Form when 10 or more items are stolen

    • The reporting member may document missing items on this form instead of their report, and it's an attachment to the offense report
    • If the victim/reporter is uncertain what's missing when the incident is reported, the reporting member leaves the form
    • The victim/reporter must be told how to complete the form, and given the appropriate sector mailing address.
  • The Affidavit of Unlawful Transient Occupancy is for when someone requests Sheriff's Office assistance to direct a transient occupant to leave a residential property.

  • The Authorization for Release of Property Form releases property back to its owner, claimant, or agent, except for vehicles, which use a Vehicle Tow Form

    • If property belonged to several claimants inventoried under one case number, prepare one Authorization for each claimant.
  • The Business Record Certification Form certifies that business records/surveillance are true and correct.

  • The Charging Affidavit Form documents each arrest with only one defendant per affidavit, but complete separate Charging Affidavits for multiple defendants on the same case. when both felony and misdemeanor offenses arise from the same criminal episode, a single Charging Affidavit Form may be used

    • Traffic misdemeanor arrests shall not be combined with other arrests on the same Charging Affidavit Form
    • A separate Charging Affidavit Form is used for juveniles.
  • The Charging Affidavit Form is used to direct file with the State Attorney's Office as follows:

    • In all adult felony or misdemeanor cases whether the defendant is in custody or at large.
    • Members shall attach the completed Charging Affidavit Form to the Incident Report page one , or signature affidavit if Spillman reporting was used, along with any other supporting documents. All documents shall be turned in to a Supervisor or designee prior to the end of the shift.
  • When a narrative is used in the Charging Affidavit Form instead of a probable statement it may serve as the narrative for the Incident Report

  • In all felony and certain misdemeanor arrests, the arresting member must complete page one of the Incident Report when handwritten or the Spillman report when the is typed on a computer

  • When the Charging Affidavit Form is used to report a traffic-related arrest, an Incident Report is not required.

  • Members shall complete each applicable block of the Charging Affidavit Form. When a physical arrest is made, copies of the Charging Affidavit Form shall be routed as follows:

  • Court Copy - Turned in at Central Booking for routing to the Clerk's Office by Dockets Section personnel.

  • Agency Copy Detached by the member and turned into the Supervisor or designee with supporting documents prior to the end of the shift, then routed to the Criminal Intake Unit.

  • Defendant's Copy – Given to the defendant or placed with the defendant's personal property.

  • If the defendant is a juvenile, members shall turn in all copies of the Charging Affidavit at the designated facility except for the Agency copy.

  • On every Child/Elder Abuse and/or Neglect report, the initial reporting sworn member must complete and fax the Child Abuse Fax Report Form to the fax number listed at the top of the form.

  • A Community Contact Card documents areas of concern in the community when sworn members make contact with citizens while patrolling

    • It can be used as a tool by citizens to provide positive feedback. The sworn member completes the card and notates areas of concern, contacts are considered consensual encounters
    • Therefore, the name of the contact person and subsequent personal information is optional. The perforated section of the card shall be completed and provided to the citizen for follow-up purposes.
  • County Ordinance Citations are used for county ordinance violations, if a UTC is not appropriate.

  • The Crime Scene Contamination Sheet documents the names of all who enter a major crime scene.

  • The Crime Scene Contamination Sheet must be attached to the Incident Report

  • The DNA Collection Consent Form is for the voluntary consent of a DNA sample collection.

  • For every domestic violence or dating violence crime reported, this form must be completed and faxed to the local domestic violence shelter fax number listed at the top of the form

    • If the victim wants confidentiality, their information (including juveniles) shall not be on the fax report summary.
    • If the victim doesn't want confidentiality, the reporting member may fax their incident narrative in lieu of completing the summary portion of the fax report.
  • Domestic Violence Worksheets shall be completed on all domestic violence and dating violence cases.

  • DUI Citations are used when arresting a defendant for DUI under FSS 316.193

    • Used where breath/blood alcohol is .08g/210L/100ML or more, or the defendant refuses testing with reason to believe they're driving under the influence.
  • The DUI Implied Consent Form is read to an arrestee to inform them of the state requirement to provide a breath sample, and the penalty if they refuse.

      • State Attorney Copy - turned in at Central Booking for routing by Dockets Section personnel to the Clerk's Office for assignment of a court case number, then forwarded to the State Attorney.
      • Driver's License Office Copy - retained by DUI Testing Center personnel for routing to the Driver's License Hearing office.
      • Defendant's Copy - given to defendant arrested for DUI.
  • The Faulty Equipment/Warning Citation is completed by entering descriptive information at the top of the form, checking the proper box, and describing the defect

    • The member notes the violated statute in the comments section.
    • The reporting member retains the white copy and gives the yellow copy to the defendant.
  • A Field Intel Card documents suspicious activities and persons, for career criminals, violent offenders, investigative targets, and sexual offenders/predators.

Incident Report

  • Prior to issuing the citation, the victim, juvenile suspect, and suspect's parent/guardian must agree to the Juvenile Civil Citation as an alternative to criminal charges
