IOTN Overview
- Purpose: To assess the impact of malocclusion on dental health and psychosocial well-being.
- Comprised of two main components: Dental Health Component (DHC) and Aesthetic Component (AC).
Dental Health Component (DHC)
- Evaluates the single worst feature of malocclusion, categorized into five grades:
- Grade 1 — No need for treatment
- Grade 2 — Little need for treatment
- Grade 3 — Moderate need for treatment
- Grade 4 — Great need for treatment
- Grade 5 — Very great need for treatment
Aesthetic Component (AC)
- Assesses the aesthetic impact of malocclusion on psychosocial aspects:
- Score 1 or 2 — None
- Score 3 or 4 — Slight
- Score 5, 6, or 7 — Moderate/borderline
- Score 8, 9, or 10 — Definite need for treatment
- Important to use in conjunction with DHC scores for a comprehensive assessment.
Treatment Eligibility in the UK
- Requires specific IOTN scores for orthodontic treatment:
- DHC = 5
- DHC = 4
- DHC = 3 AND AC > 6
MOCDO Acronym for DHC Assessment
- M - Missing Teeth (Hypodontia)
- Less extensive and extensive hypodontia impacting treatment needs.
- O - Overjet

Aesthetic Component Scoring Limitations
- Scores derived from a set of standard photographs, ranging from aesthetically pleasing to not.
- Disadvantages: Should not be used alone as a determinant; must be combined with DHC for accurate treatment need evaluation.
