Study Notes

IM Injection Sites

  • Vastus Lateralis:

    • Located on the lateral (side) middle third of the thigh, between the greater trochanter and lateral femoral condyle.
    • Locate by placing one hand on the top of the thigh, and the other hand on the knee with fingers and thumb extended. The muscle is between these two points, and any point in this area is suitable.
    • Up to 5mLs can be injected.
    • Preferred site for anaphylaxis management and infants/neonates.
  • Deltoid Muscle:

    • Located below the acromion process and above the deltoid tuberosity.
    • Locate by measuring two finger widths down from the acromion process, forming a "V" shape, and injecting in between.
    • Up to 2mLs, but 1mL is recommended.
    • Common site for vaccinations, preferred for children aged 3-18 due to more developed muscle mass.
  • Ventrogluteal:

    • Located on the side of the hip, just below the iliac crest.
    • Locate by placing thumb on the groin, pointer finger on the iliac crest, and spreading the middle finger to form a "V". The injection site is at the bottom of the "V" where fingers meet.
    • Up to 3mLs can be injected.
    • Considered one of the safest areas due to thick muscles, few blood vessels and nerves, making it less painful.
  • Dorsogluteal:

    • Located above the posterior iliac crest, to the left of the greater trochanter.
    • Locate by drawing an imaginary line between the greater trochanter and posterior superior iliac spine, and finding a point 1 inch superior to the center of this line.
    • Up to 4mLs can be injected.
    • Not recommended due to proximity to the sciatic nerve.
