Southern Nevada Fire Operations: High-Rise Operations

  • This manual outlines the incident management system, resource allocation, deployment, and tactical objectives for high-rise emergencies, as agreed upon by all Las Vegas Valley Fire Departments.
  • It focuses on ICS positions and tactical situations unique to high-rise operations and serves as a framework for the Incident Commander.

Overview of Incident Management System (IMS) Positions

  • The IMS positions include Incident Commander, Liaison, Safety, Operations, Planning, Logistics, and Information.

Incident Commander (IC)

  • The IC reviews and evaluates the Incident Action Plan (IAP) and makes necessary changes.
  • The IC provides ongoing review of the overall incident and participates in the IAP development.
  • The IC selects incident priorities and staffs Command and General Staff functions as needed.
  • The IC provides direction to Command and General Staff and reviews the organizational structure.
  • The IC establishes liaison with internal/external agencies, property owners, and tenants.
  • The IC maintains sufficient Command personnel to support the incident to its conclusion.
  • Strategic objectives include: ensuring safety, limiting loss of life and property damage, utilizing resources efficiently, ensuring fire ground communication, and ensuring orderly demobilization.

Safety Officer

  • The Safety Officer identifies existing or potential hazards and informs Command.
  • The Safety Officer recommends changes to the IAP and can alter, suspend, or terminate unsafe activities.
  • The Safety Officer investigates accidents within the incident area and requests assistance as needed.
  • The Safety Officer must have appropriate PPE, radio communication, and a partner when in hazardous positions.

Public Information Officer (PIO)

  • The PIO relays accurate information about the incident, including cause, size, current situation, and resources committed.
  • The PIO is the point of contact for media and other governmental agencies.
  • The PIO determines limits on information release and provides a link with the Joint Information Center (JIC) when established.
  • Only one PIO is designated in either a Single or Unified Command structure, but can have assistants.

Liaison Officer

  • The Liaison Officer is the point of contact for representatives from assisting or cooperating agencies (Agency Representatives).
  • The Liaison Officer coordinates the Agency Representatives.
  • The Liaison Officer participates in planning meetings and provides resource status of assisting agencies, including capabilities and limitations.

Operations Section Chief

  • The Operations Section Chief manages tactical activities and coordinates with the IC.
  • The Operations Section Chief briefs and assigns Operations personnel to support strategic objectives.
  • The Operations Section Chief implements the Operations portion of the IAP and informs Command about activities and events.
  • The Operations Section Chief assigns resources to tactical level areas based on incident objectives and strategies.
  • The Operations Section Chief builds an effective organizational structure using Branches and Divisions/Groups.

  • The Rehab Officer manages all firefighter related rehabilitation issues in Staging and maintains records of companies in Rehab, including assignment, time in, vitals, time out, and next assignment.
  • The Rehab Officer measures vitals for all personnel reporting to Rehab and reports anomalies to Fire Medical.

Fire Medical Officer

  • The Fire Medical Officer manages all firefighter related EMS issues in Staging.
  • The Fire Medical Officer evaluates personnel as requested and assists Rehab when workload permits.
  • The Fire Medical Officer maintains readiness to respond to medical emergencies.
  • The Fire Medical Officer monitors the operations channel during rapid intervention situations to coordinate patient delivery with the Rapid Intervention Team (RIT).
  • The Fire Medical Officer coordinates injured firefighter transport with the EMS Branch and reports injuries.

Accountability Officer

  • The Accountability Officer is established by the Staging Manager.
  • The Accountability Officer tracks the location and times for all units in Staging, Rehab, Fire Medical, and Operations.
  • The Accountability Officer determines when a responder is delayed, in-transit, or unaccounted for.
  • The Accountability Officer identifies and documents the entry and exit of individual responders into the hazard zone.

Equipment Officer

  • The Equipment Officer is established by the Staging Manager.
  • The Equipment Officer manages all equipment related issues in Staging and requests resources directly through Base.
  • The Equipment Officer maintains an accurate inventory list, a record of equipment ordered and delivered, prioritizes needs, and develops equipment inventory lists.

Branches

  • Branches are considered when the span of control becomes unmanageable, the incident becomes more complex, or there are distinctly different operations.
  • Branch Directors implement and control their portion of the IAP, coordinate activities within the Branch, evaluate progress, request additional resources, assign tasks to Divisions/Groups, communicate progress, and provide for safety and accountability.

Fire Branch Director

  • The Fire Branch Director manages the activities of all Divisions involved in attacking the fire and those involved in evacuation.
  • The Fire Branch Director reports to the IC or Operations (if staffed).

EMS Branch Director

  • The EMS Branch manages triage, treatment, and transport of civilian casualties.
  • The EMS Branch coordinates with the Evacuation Group and is located outside the structure, separate from Fire Medical/Rehab.
  • The EMS Branch can coordinate with Logistics/Fire Medical/Rehab for continuing treatment/transport of injured personnel during the Incident.
  • The EMS Branch manages Private Ambulance resources.

Air Ops Branch Director

  • The Air Branch manages communication with jurisdictions providing air support (rescue operations, personnel/equipment shuttling, aerial reconnaissance).
  • The Air Branch establishes suitable Landing Zones (LZ) with security personnel and coordinates all air-based activity.
  • Helicopter support may be supplied by the LVMPD, medical helicopters, and/or Air Force helicopters based at Nellis.

Divisions/Groups

  • Division/Group supervisors complete tactical assignments and account for all resources and personnel.
  • Division/Group supervisors ensure safe operations, monitor work progress, redirect activities as necessary, and coordinate actions with related activities.
  • Division/Group supervisors monitor the welfare of personnel and rotate companies through Rehab as needed.
  • Division/Group supervisors request additional resources and provide progress reports.

Investigations Group Supervisor

  • The Investigations Group obtains information and keys from building personnel and identifies the stairwell for FD personnel.
  • The Investigations Group communicates the key pick up point.
  • The Investigations Group ascends with the second company and investigates conditions on ascent and on the fire floor, checking floors below for conditions, floor plan, and occupant status.
  • The Investigations Group identifies a suitable area for Staging, hooks up to the standpipe one floor below the fire floor, and stays on room air as long as possible.
  • The Investigations Group reports the condition of the elevator lobby on the fire floor and establishes a Division on the fire floor prior to attack.

Rapid Intervention (RIT) Group Supervisor

  • The RIT group is comprised of responders equipped to rescue other emergency workers.
  • The RIT group drops off extra equipment in the staging area, orients to the floor plan, and remains in a state of readiness.
  • The RIT group leaves the high-rise hose pack un-deployed and establishes a location for deployment in the Staging Area.
  • The RIT group deploys upon order, starts searching for missing firefighters at the last known location, and coordinates victim with Fire Medical/Rehab upon egress.

Ventilation Group Supervisor

  • The Ventilation Group directs all ventilation-related activities within the structure.
  • The Ventilation Group uses the HVAC system with assistance from the Systems Control Unit.
  • The Ventilation Group breaks windows only after thorough consideration and radio coordination.
  • The Ventilation Group coordinates the use of multiple fans to pressurize stairwells and upper floors.
  • Must especially consider ventilation in the FD stairwell due to doors being blocked open.

Evacuation Group Supervisor

  • The Evacuation Group searches and evacuates all endangered or designated occupants.
  • The Evacuation Group coordinates with IC to establish a containment area for evacuees.
  • The Evacuation Group coordinates with the Systems Control Unit to use the Public Address system and directs occupants to the designated evacuation stairwell.

Logistics Section Chief

  • The Logistics Chief provides medical aid for incident personnel and manages responder rehabilitation.
  • The Logistics Chief coordinates critical incident stress debriefing, movement of injured personnel, and any needed supplies or equipment.
  • The Logistics Chief participates in IAP development, forecasts resource needs, provides fuel/repairs, obtains specialized equipment or expertise, and secures needed facilities.

Lobby Control Officer

  • The Lobby Control Officer uses the building communications system to address civilian occupants. The Systems Control Unit when implemented can facilitate this.
  • The Lobby Control Officer pressurizes stairwells with fans when the building HVAC cannot be used.
  • The Lobby Control Officer determines occupant egress to ensure a safe corridor, directing personnel to move occupants at least 200 feet from the building.
  • The Lobby Control Officer distributes and utilizes elevator phones, coordinates elevator use, assigns elevator operators, and controls access points to upper floors.
  • Lobby Control fulfills the duties of Logistics Section Chief until that position is filled.

Base Manager

  • The base is an assembly and deployment point for personnel and equipment.
  • The Base Manager verifies location of base with the Incident Commander and ensures that it is a safe distance from the building (200+ feet).
  • The Base Manager determines the most effective access, directs arriving companies, and establishes safe routes to the fire building.
  • The Base Manager maintains a log of apparatus, equipment, and personnel, coordinates movement of equipment through Lobby Control, and establishes equipment pools.
  • The Base Manager maintains a minimum of three companies in Base, requests extra personnel, controls entry/exit, secures the perimeter (utilizing PD if necessary), and keeps companies together.
  • The Base Manager parks apparatus for free movement, assembles arriving members, advises command of availability, and supplies auxiliary water to the stairwell entry point if needed.

Ground Support Manager

  • Ground Support is implemented when equipment needs to be moved to Staging via stairwells or when an additional water supply is needed.
  • The Ground Support Manager refills SCBA cylinders, coordinates equipment transport to the staging floor, and coordinates with Base to get auxiliary water to the stairwell entry point.
  • The Ground Support Manager posts a firefighter every two floors to establish an equipment shuttle.
  • The Ground Support Manager secures the wye and hose when placing an auxiliary water supply through the stairwell’s center.
  • The Ground Support Manager ensures personnel have PPE (including SCBA) and provides auxiliary lighting if necessary.

Communications Unit Leader

  • The Communications Unit Leader ensures effective communication between the Incident Commander and incident personnel.
  • This includes portable and cellular radios and the building's sound-powered system, and spare batteries.

Planning Section Chief

  • The Planning Section Chief serves as the IC’s clearinghouse for information, planning ahead and identifying resource needs.

Resource Unit

  • The Resource Unit tracks information related to the control of resources, including workforce, apparatus, and agencies on scene.
  • Maintains a master check-in list of personnel and resources

Situation Unit

  • The Situation Unit collects and updates information regarding weather, traffic, or any other relevant data.
  • Collects and updates information including Current status of the Incident, Forecast status of the Incident.

Documentation Unit

  • The Documentation Unit provides a comprehensive and chronological record of the Incident activities, including reports, logs, injury reports, situation status reports, and post-incident analysis.

Technical Specialists Unit

  • The Technical Specialists Unit coordinates activities and technical advice from agencies such as building engineers, elevator company representatives, structural engineers, and architects.

Finance/Administration Section Chief

  • The Finance/Administration Section Chief procures services and supplies, documents all financial costs, and documents for possible cost recovery of services and/or supplies.
  • They also analyze legal issues and document compensation and claims of injury.

Order of Assignment

  • This section addresses the standard order of assignment for Incident Management and Tactical positions based on order of arrival, but the Incident Commander can deviate from this plan based on incident needs.

Typical First-Alarm Assignment

  • The assignment includes 5 Engines, 2 Trucks, 1 Heavy/Engine, 3 Rescues, and 2 Battalion Chiefs.
  • Company #1 conducts size-up, establishes Command, and designates the Base channel, and becomes the Investigations Group.
  • Companies #2-4 assemble equipment, ascend to the fire floor, and assist with fire attack.
  • Company #5 establishes Lobby Control.
  • Company #6 ascends to the floor above the fire for recon and evacuation, becoming a Division.
  • Company #7 establishes staging two floors below the fire floor.
  • Company #8 reports through staging to take up RIT position.
  • Rescue #1 assumes the role of Systems Control Unit.
  • Rescue #2 and #3 gather necessary equipment and ascend to Staging to assume Fire Medical.
  • Battalion #1 assumes Command and establishes a Command Post.
  • Battalion #2 assumes the Deputy IC role.

Recycle/On Deck System

  • Critical for high-rise incidents to maintain uninterrupted water flow.
  • Division Supervisors use the "On Deck / Recycle" system to maintain a steady flow of water.
  • Crews rotate through the nozzle while fresh crews are brought in, replace cylinders and rehydrate.

Typical Second Alarm Upgrades

  • The upgrades include 5 Engines, 2 Trucks, 1 Heavy/Engine, 3 Rescues, 2 Battalion Chiefs, and an Air Resource Unit.
  • The first arriving company establishes Base.
  • Incident Commanders deploy resources based on tactical needs.
  • Four companies go to Staging, two companies establish the Evacuations Group, one company supplements RIT, and one company supplements Lobby.
  • Rescues can be used to staff the EMS Branch.
  • Battalion #1 leads Fire Branch, Battalion #2 is Deputy for the Fire Branch Director, and Battalion #3 & 4 assume Incident Command and Logistics Section.

Typical 3rd and Subsequent Alarm Upgrades

  • The upgrades include 3 Engines, 1 Truck, and 1 Rescue.
  • Incident Commanders deploy resources based on the tactical needs of the incident.
  • Battalion #3 may assume leadership of the Operations (Ops) Section.
  • Battalion #4 may assume the role of Deputy Operations Chief.
  • Deputy Chief #1 assumes Command.

Communications Plan

  • The Communications Plan improves overall communications within the Incident, outlining who will talk with whom and on which channels.
  • Second alarm and subsequent units respond on the Base Channel.
  • The flow of Personnel and Equipment through control points takes place on the Logistics (Log) Channel.
  • Additional channels may be necessary for different Divisions and Groups.

Communications Plan for First Alarm Units by Assignment

  • Company #1 establishes Command and then establishes the appropriate Division on the fire floor.
  • Companies #2-4 are assigned to Investigations/Division.
  • Company #5 establishes Lobby Control.
  • Company #6 establishes the appropriate Division on the floor above the fire.
  • Company #7 establishes the Staging area.
  • Company #8 establishes RIT.
  • Rescue #1 establishes the Systems Control Unit.
  • Rescue #2 establishes Fire Medical in the staging area.
  • First Full Engine Company establishes Base.
  • Evacuation Group maintains communications with the IC, Ops, or Fire Branch on the Ops channel.
  • EMS Branch establishes the EMS Branch on channel designated.

Evacuations

  • Evacuation requires a coordinated effort.
  • Select a stairwell dedicated to evacuation, announcing it via radio and the public address system.
  • Coordinate with Command to determine the marshalling point for evacuees, either a safe location within the building or outside the building at least 200’ from any falling debris.
  • Considerations include beginning in the immediate fire area, directing evacuees to an area of safe refuge, and concentrating on stairways, hallways, and elevator lobbies above the fire floor.
  • Utilize the PA system in the fire control room.

Primary Search Markings

  • Markings include a forward slash on the door, the unit number approximately 1/3 of the way down from the top of the forward slash.

Secondary Search Markings

  • Secondary search markings include any additional live victims are found, the secondary search team will make a notation indicating where they have been evacuated to.

Firefighter Injury/Illness

  • SNFO Mayday and Emergency Traffic procedures shall be followed based upon the urgency of the situation.

Accountability

  • Accountability is everyone’s responsibility.
  • Passports provide no benefit if they become trapped or missing along with the people in trouble.
  • The passport system identifies exactly who is trapped or missing.
  • Command/Operations/Branches maintain strategic accountability, tracking crews and Divisions/Groups by location and function on a tactical worksheet.
  • Division/Group supervisors are responsible for tactical accountability.

Tactical Worksheets

  • Tactical Work Sheets should record the following data:
  • Resource status
  • Resource assignment
  • Resource-tracking accountability
  • Simple drawing of the incident area or occupancy and assigned resources
  • Tactical priority checklist and a list of standard benchmarks for completion

Personnel Accountability Report (PAR)

  • The Personnel Accountability Report (PAR) involves accounting for all personnel assigned to crews and Division/Groups.
  • The PAR is a confirmation that all members are accounted for and have an adequate exit air supply.

Incident Command Staffing and Communications Plan

  • Battalion Chief #1 takes Command (Ops Channel) from the initial IC.
  • Battalion Chief #2 assumes the Deputy/Logistics role.
  • IC establishes the Logistics Channel upon arrival of the Deputy IC (BC #2).
  • At the very least, the IC will need to communicate either face-to-face, or via radio (Command Channel) with the following units:
  • Fire Branch
  • EMS Branch
  • Logistics
  • Dispatch Center

Incident Command Checklist

  • Strategic Objectives of the Incident Commander include:
  • Ensure safety of emergency personnel and public
  • Limit loss of life
  • Limit property damage
  • Utilize resources efficiently and effectively
  • Ensure adequate fire-ground communication
  • Ensure prompt and orderly demobilization
  • Command must staff and position personnel in the command area to assist with managing communications on the Ops channel, Logs channel, EMS channel, and Command channel (if established)

  • It is important for those initial crews on the fire floor to understand that this plan is designed to support the placement of a single handline.

Staging Checklist

  • The staging area is a marshalling point (usually 2 floors below the fire) for equipment and manpower to facilitate quick and easy deployment to the fire area.
